Post-fistula surgery
Hello, doctor.
I had a fistula surgery about a month ago.
Two weeks ago, the surgeon told me that I no longer needed to take sitz baths and that I didn't need to return for follow-up appointments, just to apply medication.
However, it has been over a month now, and the wound still has a light yellowish-white discharge that is thick.
I also experience pain and itching after using the restroom, and occasionally there is some oozing of blood when I apply the medication.
I would like to know if this situation could indicate an infection, and if I need to return for a follow-up appointment to confirm this.

After anal fistula surgery, it is common to have wound discharge that may last for 1-2 months.
Taking warm sitz baths should help.
If the wound does not show improvement, it is advisable to return to the physician for a thorough examination.

After undergoing fistula surgery, it is not uncommon to experience some discharge from the surgical site, especially in the initial weeks following the procedure. However, the characteristics of the discharge, along with any accompanying symptoms, can provide important clues regarding the healing process and the potential for infection.
In your case, the presence of a thick, yellowish-white discharge, along with pain and itching, raises some concerns. While some discharge can be normal as the body heals, particularly in the case of a fistula, the consistency and color of the discharge can indicate whether there is an underlying issue, such as an infection. Typically, a healthy healing wound may produce a small amount of clear or slightly colored fluid, but a thick, purulent discharge (especially if it is yellow or green) can suggest infection.
The pain and itching you are experiencing, particularly after bowel movements, can also be indicative of irritation or infection at the surgical site. It is important to monitor these symptoms closely. If the pain worsens, if you notice an increase in the amount or change in the color of the discharge, or if you develop fever or chills, these could be signs of a more serious infection that requires medical attention.
Regarding your question about whether you should return for a follow-up appointment, it is advisable to consult your surgeon or healthcare provider. Even if they initially indicated that a follow-up was not necessary, your ongoing symptoms warrant a reassessment. A healthcare professional can perform a thorough examination to determine if there is an infection or if the healing process is proceeding as expected. They may also provide additional treatment options, such as antibiotics if an infection is confirmed.
In terms of wound care, it is crucial to keep the area clean and dry. If you have been advised to stop sitz baths, consider gentle cleansing with mild soap and water, and ensure that the area is thoroughly dried afterward. Applying any prescribed topical medications as directed is also important. Avoiding irritants, such as harsh soaps or lotions, can help minimize discomfort.
In summary, while some discharge can be normal after fistula surgery, the characteristics of your discharge, along with the pain and itching you are experiencing, suggest that it would be prudent to seek further evaluation. Early intervention can help prevent complications and promote a smoother recovery. Always trust your instincts regarding your health, and do not hesitate to reach out to your healthcare provider for guidance.
