Post-nasal drip for many years?

Hello, doctor.
I have been experiencing post-nasal drip since childhood, for over ten years now.
I previously underwent surgery for turbinate hypertrophy and a deviated septum.
However, I still have a constant runny nose every day, which severely affects my sleep quality.
What troubles me even more is that sometimes my nose emits a foul odor that others can also smell, which is very distressing.
Additionally, I cannot sleep on my back; as soon as I fall asleep, I snore, and it feels like my breathing is obstructed, causing me to wake up immediately.
I also cannot turn to my right side while sleeping because my throat becomes very tight and itchy, leading to coughing and difficulty breathing, as the right side of my neck feels swollen compared to the left.
I generally feel fatigued, with dark circles under my eyes, and I always feel weak and tired.
I would like to ask the doctor if nasal sprays have any side effects.
Furthermore, I came across a procedure called nasal nerve ablation online; what are the risks associated with this surgery? My nasal allergy issues have been troubling me for a long time, so I would like to know what I should do.

Hello, nasal drip may be related to nasal allergies.
It is recommended to use oral antihistamines or add a nasal corticosteroid spray for treatment, which generally works well.
Nasal corticosteroid sprays are quite safe for long-term use and do not have systemic side effects.
As for the surgical procedure of sphenopalatine ganglion nerve resection, there are not many significant side effects; however, a few individuals may experience dryness in the eyes due to the connection between the sphenopalatine ganglion and the lacrimal glands.
Cutting the nerve can affect the secretion of the lacrimal glands.
Wishing you peace and health.
Sincerely, Dr.
Jue-Yi Cheng, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Chronic nasal drip, often referred to as post-nasal drip, can be a frustrating and persistent issue for many individuals. Based on the symptoms you've described, including the foul odor from your nose, difficulty sleeping, and the sensation of nasal congestion, it seems that your condition may be multifactorial, involving both anatomical and possibly allergic components.

Understanding Your Symptoms
1. Post-Nasal Drip: This occurs when excess mucus accumulates in the back of the throat, leading to a sensation of dripping. It can be caused by various factors, including allergies, sinus infections, or structural issues in the nasal passages.

2. Nasal Odor: The unpleasant smell you mentioned could be due to chronic sinusitis or nasal infections. When mucus stagnates, it can become a breeding ground for bacteria, leading to foul-smelling secretions.

3. Sleep Disturbances: The inability to sleep on your back without experiencing snoring or a sensation of obstruction suggests that you may have obstructive sleep apnea (OSA). This condition is characterized by repeated episodes of partial or complete blockage of the airway during sleep, leading to disrupted sleep and daytime fatigue.

4. Neck Swelling: The swelling you feel on one side of your neck could indicate lymphatic involvement or inflammation, possibly related to chronic sinus issues or allergies.

Treatment Options
1. Nasal Sprays: Nasal sprays, particularly corticosteroid sprays, can be effective in reducing inflammation and mucus production. However, they can have side effects, including nasal irritation or, in rare cases, nasal septum perforation if used excessively. It’s essential to use them as directed by your physician.

2. Antihistamines: If allergies are a contributing factor, antihistamines can help reduce mucus production and alleviate symptoms. However, some antihistamines can cause drowsiness, which may not be ideal if you're already experiencing fatigue.

3. Saline Irrigation: Regular saline nasal irrigation can help clear mucus and allergens from the nasal passages, providing relief from congestion and post-nasal drip.

4. Surgery: You mentioned nasal nerve ablation, which is a procedure that can help reduce mucus production by targeting the nerves responsible for stimulating mucus glands. While this procedure can be effective, it does carry risks, including infection, bleeding, and the possibility of altered sensation in the nasal area. It's crucial to discuss these risks with your ENT specialist.

5. Sleep Positioning: To manage your sleep apnea symptoms, consider using a wedge pillow to elevate your head while sleeping. This can help reduce the likelihood of airway obstruction and improve your sleep quality.

6. Lifestyle Modifications: Staying hydrated, avoiding known allergens, and maintaining a clean environment can help manage your symptoms. Additionally, consider consulting with an allergist for comprehensive allergy testing and management strategies.

Next Steps
Given the complexity of your symptoms, it would be beneficial to have a thorough evaluation by an ENT specialist who can assess your nasal anatomy and any underlying conditions contributing to your chronic nasal drip. Imaging studies, such as a CT scan of the sinuses, may be warranted to identify any structural abnormalities or chronic sinusitis.

In summary, while there are various treatment options available for chronic nasal drip and associated symptoms, a tailored approach based on a comprehensive evaluation is essential. Working closely with your healthcare providers will help you find the most effective strategies to manage your condition and improve your quality of life.
