Questions regarding the use of medications for Pilonidal Disease?

Hello, Doctor:
1.
Due to previously elevated IgM levels, my immunologist prescribed Belatacept, two tablets daily.
I have been taking it for three months; while my levels have not increased, they also have not decreased.
What could be the possible reasons for this?
2.
Belatacept is a long-term prescription medication.
Sometimes I notice a bitter taste this month, and sometimes I don’t the next month.
Which is normal? Or could there be an issue with my taste perception?
3.
Before prescribing the long-term medication, my immunologist asked me to take it for two weeks to check for any side effects, such as rashes.
Initially, I did not experience any, but this past month I have been itching and developed hives.
Could this be a side effect of Belatacept? If so, should I return for a medication change? If not, can I use dermatological treatments in conjunction with it?
Thank you for your patient response.

Dr. Huang Shenggang reply Pharmacology

Hello Sarah,
Thank you for your question.

1.
The indications for the medication Belimumab are as follows:
1.
Discoid and systemic lupus erythematosus.

2.
Chronic polymorphous light eruption.

3.
Chronic rheumatoid arthritis.

4.
Malaria caused by Plasmodium falciparum and Plasmodium vivax.

Regarding the elevated IgM levels that have neither increased nor decreased, this issue should be discussed with your attending physician.
Sometimes, the effects of the medication can alleviate symptoms without necessarily causing changes in laboratory values, so a comprehensive understanding and detailed diagnostic testing are needed.
2.
Common side effects of Belimumab are primarily related to the skin, and some users may experience decreased appetite.
For any taste-related issues, you may need to discuss this with your physician.
3.
As mentioned in the second paragraph, side effects may not manifest immediately; they can sometimes appear after prolonged use.
If you experience skin itching or rashes, please inform your physician to see if an alternative medication can be prescribed.
If necessary, you may also consult a dermatologist for further evaluation and diagnosis.
Thank you again for your inquiry.

More Info

Bylakrid, known generically as Belimumab, is a monoclonal antibody used primarily in the treatment of systemic lupus erythematosus (SLE). It works by inhibiting the activity of B-lymphocyte stimulator (BLyS), a protein that plays a crucial role in the survival of B cells, which are responsible for producing antibodies. While it can be effective in managing lupus symptoms and reducing disease activity, patients often have concerns regarding its efficacy and potential side effects.

1. Efficacy and Lack of Improvement
You mentioned that after three months of treatment with Bylakrid, your IgM levels have not increased or decreased. This could be due to several factors:
- Individual Variability: Each patient's response to medication can vary significantly. Factors such as genetics, the severity of the disease, and concurrent medications can influence treatment outcomes.

- Disease Complexity: Lupus is a complex autoimmune disease with various manifestations. Sometimes, even with treatment, certain biomarkers may not change significantly while the patient's overall condition may still improve.

- Time Frame: Three months may not be sufficient to observe significant changes in IgM levels. Some patients may require a longer duration of therapy to see measurable changes.

It is essential to maintain open communication with your healthcare provider regarding your treatment progress and any concerns you may have.

2. Taste Variation
Regarding the variable taste of the medication, it is not uncommon for patients to experience fluctuations in taste perception. This can be influenced by several factors:
- Medication Formulation: Sometimes, the formulation of the medication can change slightly between batches, which might affect taste.

- Physiological Changes: Changes in your body, such as hydration levels, oral health, or even changes in your diet, can affect how you perceive taste.

- Medication Side Effects: Some medications can alter taste perception, leading to a bitter or metallic taste. If this persists or worsens, it would be advisable to discuss it with your physician.

3. Skin Reactions and Urticaria
The development of hives (urticaria) and itching after starting Bylakrid could potentially be a side effect of the medication. While not all patients experience side effects, skin reactions can occur. Here are some considerations:
- Allergic Reaction: Hives can be a sign of an allergic reaction to the medication. If you suspect this is the case, it is crucial to contact your healthcare provider promptly.

- Management Options: If the urticaria is indeed related to Bylakrid, your doctor may recommend antihistamines or other treatments to manage the symptoms. In some cases, they may consider switching medications if the side effects are significant.

- Consulting a Dermatologist: If the itching and hives persist, consulting a dermatologist may provide additional treatment options and help determine if the skin reaction is related to the medication or another cause.

Conclusion
In summary, while Bylakrid can be effective for managing lupus, individual responses can vary, and side effects such as taste changes and skin reactions may occur. It is essential to maintain regular follow-ups with your healthcare provider to monitor your condition and adjust treatment as necessary. If you experience significant side effects or have concerns about your treatment, do not hesitate to reach out to your healthcare team for guidance. They can provide personalized advice and support tailored to your specific situation.
