Testicular texture
Hello, Doctor Du.
I would like to ask you about my left testicle, which feels much softer in texture compared to my right testicle (at the time, the scrotum was relaxed).
Is this considered normal? I have been experiencing some soreness in the testicles, so I have visited the hospital for consultation five times.
The doctor mentioned that there is a 0.1 cm cyst on the left epididymis, but said it would not have any significant impact.
When I asked the doctor why this might be happening, he suggested it could be related to prolonged sitting, walking, or standing.
However, I haven't been sitting, walking, or standing for long periods.
I still experience soreness; what could be the reason for this? Does a decrease in immunity also affect the firmness of the testicles? What is the ideal texture of the testicles to be considered not too soft?

Hello: The two testicles do not necessarily have the same firmness; there can be some differences.
Testicular hardness may indicate the possibility of varicocele.
If the urology department at a medical center has stated that everything is normal, there is no need to worry too much.
If there are symptoms causing significant discomfort, medication can be taken.
Immunity will not be affected.
The texture should be softer than a boiled egg and should have a slight elasticity.

When it comes to testicular health, understanding the texture and any accompanying symptoms is crucial for identifying potential issues. Testicular softness, especially when compared to the other testicle, can be a cause for concern, particularly if accompanied by pain or discomfort.
In your case, you mentioned that your left testicle feels significantly softer than the right and that you experience a sensation of soreness. While some degree of asymmetry in texture can be normal, a notable difference, especially if it is accompanied by pain, warrants further investigation. The presence of a small cyst on the epididymis, as noted by your doctor, is often benign and may not directly contribute to the symptoms you are experiencing. However, the discomfort and the change in texture should not be overlooked.
Several factors can influence testicular texture and health. For instance, prolonged sitting, standing, or walking can lead to discomfort in the groin area, but if you do not engage in these activities frequently, it may not be the primary cause of your symptoms. Other potential factors include hormonal imbalances, infections, or even trauma to the area that may not have been immediately apparent.
Regarding your question about immunity, while a weakened immune system can lead to various health issues, it is not typically associated with changes in testicular firmness directly. However, systemic infections or conditions that affect overall health can indirectly impact testicular health.
In terms of what constitutes a "normal" texture for the testicles, they are generally firm but not hard, with a smooth surface. A healthy testicle should feel similar to a firm rubber ball. If the texture is significantly softer than this, it could indicate a potential issue, such as fluid accumulation (hydrocele), varicocele, or even testicular atrophy, which can occur due to various reasons including hormonal changes or chronic conditions.
Given your ongoing symptoms and the fact that you have sought medical attention multiple times, it may be beneficial to request further diagnostic tests, such as an ultrasound, to assess the structure of the testicles and surrounding tissues more thoroughly. This imaging can help rule out any underlying conditions that may not be evident during a physical examination.
In summary, while some variation in testicular texture can be normal, significant differences, especially when accompanied by pain or discomfort, should be taken seriously. It is essential to maintain open communication with your healthcare provider and advocate for further evaluation if your symptoms persist or worsen. Regular monitoring and proactive management are key to maintaining reproductive health and addressing any potential issues early on.
