Testicular consistency (soft or hard)
Hello, doctor.
This is my first time asking a question on this platform, and I apologize if I offend in any way.
I have been troubled by a couple of issues recently: (1) The firmness of my testicles is not the same on both sides; the left side is slightly softer while the right side is a bit harder than the left (but both have elasticity).
Should I be concerned about this? (2) Also, when I take a shower, I gently press on my testicles to check for any lumps, and I noticed that after pressing on the right testicle, I experience soreness in my right buttock for a while.
Is there anything I should be aware of regarding this? Thank you, doctor, for taking the time to read this.

Self-examination is a good practice; however, if you notice any abnormalities, it is advisable to schedule a scrotal ultrasound examination with a urologist for further evaluation.

More Info
When it comes to testicular health, it's essential to be vigilant and aware of any changes in the consistency or sensation of the testicles. Your concerns about the differing hardness of your testicles and the discomfort you experience when pressing on the right testicle are valid and warrant further discussion.
1. Differences in Testicular Hardness: It is not uncommon for one testicle to feel slightly different in hardness compared to the other. The testicles can have variations in texture and firmness due to several factors, including normal anatomical differences, the presence of epididymal fluid, or even the state of the surrounding tissues. However, significant changes in hardness, especially if accompanied by pain, swelling, or the presence of lumps, should be evaluated by a healthcare professional.
2. Discomfort in the Right Testicle: The sensation of discomfort or pain in the buttock area after pressing on the right testicle could indicate a few things. It might be related to referred pain from the testicle, which can occur due to the close anatomical relationship between the testicles and the surrounding structures, including nerves that may transmit pain signals. Another possibility could be related to the epididymis, which is the tube that carries sperm from the testicle and can sometimes become inflamed or irritated.
When to Seek Medical Advice
You should consider seeking medical advice if you experience any of the following:
- Persistent Changes: If the difference in hardness becomes more pronounced or if you notice any new lumps, bumps, or changes in size.
- Pain: If you experience persistent pain in the testicle or the surrounding areas, including the abdomen or lower back.
- Swelling: Any noticeable swelling or enlargement of one testicle compared to the other.
- Discharge: Any unusual discharge from the penis or changes in urination.
- Systemic Symptoms: Fever, chills, or other systemic symptoms that accompany the discomfort.
Importance of Regular Self-Examinations
It is good practice to perform regular self-examinations of your testicles. This can help you become familiar with their normal size and texture, making it easier to notice any changes. When performing a self-exam, you should gently roll each testicle between your fingers and look for any lumps or abnormalities.
Conclusion
While slight variations in testicular hardness can be normal, it is crucial to remain attentive to your body and any changes that occur. If you have concerns about the differences in hardness or the discomfort you are experiencing, it is advisable to consult a healthcare provider, preferably a urologist. They can perform a thorough examination, possibly including an ultrasound, to rule out any underlying conditions such as testicular torsion, epididymitis, or even tumors. Early detection and intervention can significantly improve outcomes in any potential testicular health issues.
Remember, your health is paramount, and seeking professional advice is always a wise decision when it comes to changes in your body.
