Regarding pain above the left eye?

Hello Doctor: I work in soldering electronic components, with the size of the components being approximately 0.4mm.
On November 4, 2019, I felt a foreign body sensation above my left eye (I had been playing mobile games for about a week prior).
On November 6, 2019, I visited an ophthalmologist who diagnosed me with eyelid inflammation, and my intraocular pressure was normal.
On November 9, 2019, I went to another ophthalmologist, and again my intraocular pressure was normal; the doctor also stated it was inflammation.
By November 19, during this time, I noticed that the foreign body sensation above my left eye had disappeared, but I experienced intermittent soreness, sometimes shifting to the lower left side and occasionally to the upper right side, though it was not constant.
I would like to ask: 1.
Could this be glaucoma? 2.
Can the eye only hurt in the upper half? 3.
I cannot distinguish whether the pain is in the eyeball or the surrounding eye muscles.
4.
Does this count as eye strain? Thank you, Doctor.

Xiao Lai, 30~39 year old female. Ask Date: 2019/11/19

Hello Mr.
Lai: Your intraocular pressure is normal, so the soreness should not be caused by it.
The doctor has examined you and should be able to rule out glaucoma.
If there is a sensation of a foreign body without any actual foreign object present, the most common causes are dry eyes or inflammation.
If you still experience soreness and have been using your eyes excessively, it could be muscle strain.
Muscle strain can occur around the eyes, and it can happen in any area.
Please allow your eyes to rest more, and try to avoid playing mobile games outside of work to see if your condition improves.

Understanding pain above the left eye can be quite complex, as it may stem from various underlying conditions. Based on the symptoms you've described, including the sensation of an object in your eye, inflammation, and the shifting pain around your eye, several potential causes can be considered.

1. Inflammation: As noted in your previous visits to the ophthalmologist, inflammation of the eyelid (blepharitis) or conjunctiva (conjunctivitis) can lead to discomfort and a feeling of pressure or pain in the eye area. This inflammation can cause the surrounding muscles to become tense, leading to referred pain in different areas around the eye, including the upper and lower parts.

2. Eye Strain: Given your occupation involves welding small electronic components, it is likely that you are experiencing eye strain, especially if you are working for extended periods without adequate breaks. This can lead to symptoms such as discomfort, fatigue, and pain around the eyes. The use of screens, particularly if you have been playing mobile games, can exacerbate this issue, leading to digital eye strain.

3. Sinus Issues: The sinuses are located near the eyes, and sinusitis can cause pain that feels like it is coming from the eye area. If you have had any upper respiratory symptoms or allergies, this could be a contributing factor.

4. Migraine or Tension Headaches: Migraines can cause localized pain around the eyes, and tension headaches can lead to muscle tightness around the forehead and eyes, resulting in pain that may feel like it is coming from the eye itself.

5. Glaucoma: While you have mentioned that your eye pressure was normal during your visits, it is important to note that glaucoma can sometimes present with pain, especially if there is an acute angle-closure attack. However, this is less likely given your normal eye pressure readings.

Now, addressing your specific questions:
1. Could this be glaucoma?: Given that your eye pressure has been consistently normal during examinations, it is unlikely that you are experiencing glaucoma. However, if you have concerns about your risk factors or family history, it would be prudent to discuss this further with your ophthalmologist.

2. Can the eye ball only hurt in the upper half?: Yes, pain can be localized to specific areas of the eye or surrounding tissues. This can be due to muscle strain, inflammation, or referred pain from other structures.

3. Is the pain from the eye itself or surrounding muscles?: Distinguishing between pain from the eye and pain from surrounding muscles can be challenging. If the pain worsens with eye movement or is associated with visual disturbances, it may indicate an issue with the eye itself. Conversely, if the pain is more generalized and related to muscle tension, it may be muscular.

4. Is this due to eye strain?: Yes, your symptoms could very well be a result of eye strain, especially considering your work environment and recent activities. It is essential to take regular breaks, practice the 20-20-20 rule (every 20 minutes, look at something 20 feet away for 20 seconds), and ensure proper lighting and ergonomics while working.

In conclusion, while your symptoms may not indicate a severe condition, it is crucial to monitor them closely. If the pain persists or worsens, or if you experience any changes in vision, it is advisable to seek further evaluation from an eye care professional. They may recommend additional tests or treatments, such as anti-inflammatory medications, warm compresses, or lifestyle adjustments to alleviate your symptoms. Taking care of your eye health is essential, especially in a demanding work environment.
