Loss of sensation in the penis?
Four months ago, I accidentally injured my penis, resulting in loss of sensation.
When I saw the doctor, he said there were no visible issues (I personally noticed that the front half of the glans appears paler than before and has some bruising; it sometimes feels cold to the touch).
He ordered a pelvic MRI, which came back completely normal.
The doctor mentioned that it might take about six months to recover, but after this time, there hasn't been much improvement (even my testicles seem to be affected and have lost some sensation, and my libido is lower than before).
I would like to inquire about what could be causing this.
Could there be internal injuries within the penis?

If the MRI results are normal, then there is no issue; it is likely a psychological factor.

More Info
Penile numbness following an injury can be a concerning and distressing condition. Based on your description, it seems that you experienced a significant trauma to the penis four months ago, resulting in loss of sensation and changes in appearance. The fact that your doctor conducted a pelvic MRI and found no abnormalities is a positive sign, but it does not completely rule out the possibility of nerve damage or other underlying issues.
Causes of Penile Numbness
1. Nerve Injury: The penis is richly supplied with nerves, and trauma can lead to nerve compression or damage. This can result in numbness or altered sensation. The nerves may take time to heal, and in some cases, they may not fully recover.
2. Vascular Compromise: Trauma can also affect blood flow to the area. If the blood vessels are damaged or compressed, it can lead to changes in color (such as paleness) and sensation. The bruising you mentioned could indicate some level of vascular injury.
3. Psychological Factors: Sometimes, psychological factors such as anxiety or stress related to the injury can also contribute to changes in sexual function and sensation. This is particularly relevant if you are experiencing a decrease in libido.
4. Underlying Conditions: Conditions such as diabetes or other metabolic disorders can also affect nerve health and sensation. If you have any pre-existing conditions, they could be contributing to your symptoms.
Recovery and Management
1. Time: Nerve healing can be a slow process. While your doctor mentioned that improvement could occur within six months, it is essential to understand that recovery timelines can vary significantly among individuals. Some may experience gradual improvement, while others may take longer.
2. Physical Therapy: Engaging in pelvic floor physical therapy may help in some cases. A specialized therapist can work with you to improve blood flow and nerve function in the area.
3. Medication: If you are experiencing pain or discomfort, medications such as NSAIDs (non-steroidal anti-inflammatory drugs) may help. In some cases, medications specifically targeting nerve pain, such as gabapentin or pregabalin, may be considered.
4. Follow-Up: It is crucial to maintain regular follow-up appointments with your healthcare provider. If your symptoms persist or worsen, further evaluation may be necessary. This could include additional imaging studies or consultations with specialists such as a urologist or neurologist.
5. Lifestyle Modifications: Reducing stress through relaxation techniques, exercise, and a healthy diet can also support overall recovery. Avoiding activities that may exacerbate your symptoms is advisable.
Conclusion
In summary, penile numbness following trauma can be attributed to nerve injury, vascular issues, or psychological factors. While the MRI results are reassuring, it is essential to continue monitoring your symptoms and maintain open communication with your healthcare provider. Recovery can take time, and various therapeutic approaches may assist in regaining sensation and function. If you experience any new symptoms or significant changes, seek medical attention promptly. Your health and well-being are paramount, and addressing these concerns with a professional will provide the best path forward.
