Nipple issues?

Hello doctor, I would like to inquire about an issue.
I am 21 years old, and my right nipple is more prominent.
It hurts when pressed, and sometimes it itches.
If I press on it, it bleeds from a small hole.
I can see small blood vessels on my right nipple, while the left nipple is normal.
Due to the prominence, wearing thinner bras causes irritation, so I have to use tissue for padding.
I also haven't had my period for a long time.
In middle school, I suddenly gained weight from 60 kg to almost 80-90 kg within a year, and at that time, my nipples were not like this; it started around high school.

More Info

It sounds like you're experiencing some concerning symptoms related to your right nipple, including pain, itching, and bleeding from a small hole. Given your age and the nature of these symptoms, it's important to address them with a healthcare professional as soon as possible. Here’s a breakdown of what might be happening and some potential causes.

Possible Causes of Nipple Issues
1. Infection or Inflammation: Conditions such as mastitis or an abscess can cause pain, swelling, and discharge from the nipple. These infections can occur in lactating women but can also affect non-lactating individuals. Symptoms may include redness, warmth, and tenderness in the area.

2. Nipple Trauma: If your nipple is protruding more than usual, it may be more susceptible to trauma from clothing or friction, especially if you wear thin fabrics. This can lead to irritation, bleeding, and even the formation of small wounds.

3. Dermatitis: Skin conditions like eczema or contact dermatitis can cause itching and discomfort around the nipple area. This could be exacerbated by certain fabrics or hygiene products.

4. Duct Ectasia: This condition occurs when a milk duct beneath the nipple dilates, fills with fluid, and thickens. It can lead to a sticky discharge and sometimes pain or tenderness.

5. Fibrocystic Changes: Many young women experience fibrocystic breast changes, which can cause lumps, tenderness, and sometimes changes in the nipple. These changes are usually benign but can be uncomfortable.

6. Hormonal Changes: Since you mentioned irregular menstrual cycles, hormonal fluctuations could be contributing to breast tenderness and changes in the nipple. Hormones can affect breast tissue and lead to various symptoms.

7. Paget's Disease of the Nipple: Although rare, this condition can cause changes in the skin and appearance of the nipple, including flaking, itching, and discharge. It is often associated with underlying breast cancer, so it’s crucial to rule this out.

Recommendations
1. Consult a Healthcare Provider: Given the bleeding and the presence of a hole in your nipple, it is essential to see a doctor, preferably a dermatologist or a breast specialist. They can perform a physical examination and may recommend imaging studies like an ultrasound or mammogram to assess the underlying structures.

2. Avoid Irritation: Until you can see a doctor, try to minimize irritation to the area. Consider wearing a supportive bra that reduces friction and protects the nipple. Avoid tight clothing that may exacerbate the issue.

3. Monitor Symptoms: Keep track of any changes in your symptoms, such as the frequency of bleeding, changes in color or texture of the nipple, or any new lumps in the breast. This information will be helpful for your healthcare provider.

4. Maintain Hygiene: Ensure that the area is clean and dry. If you notice any discharge, you may want to gently clean the area with mild soap and water.

5. Consider Hormonal Factors: Since you mentioned irregular periods and weight changes, it might be beneficial to discuss these aspects with your healthcare provider. Hormonal imbalances can affect breast tissue and lead to various symptoms.

Conclusion
Your symptoms warrant a thorough evaluation by a healthcare professional to determine the exact cause and appropriate treatment. While some conditions may be benign, others could require medical intervention. Don't hesitate to seek help, as early diagnosis and treatment can significantly improve outcomes and alleviate discomfort. Remember, your health is paramount, and addressing these issues promptly is crucial.
