After clavicle surgery, there is shoulder stiffness?
Hello Doctor, I hope you don't mind me asking.
After my clavicle fracture surgery on December 2nd last year, I have been using a triangular bandage for support.
On January 6th of this year, I returned for a follow-up and the doctor said I could remove it.
However, since taking it off (it's been 3 weeks now), I've been feeling stiffness and a sensation of tightness in my shoulder.
I have been actively moving it during the day, but my condition hasn't improved.
I'm wondering if this is a normal phenomenon after clavicle surgery or if it is something abnormal.
Thank you.

Shoulder stiffness after clavicle fracture surgery is a normal phenomenon.
You can start performing movements to mobilize the shoulder joint.
Initially, you can do forward and backward swings.
After six weeks, you can begin overhead movements.
It is only after twelve weeks that you can try lifting heavy objects.
Gradual progression is key to recovery.

More Info
Shoulder stiffness after clavicle surgery is a common concern among patients, and it can be attributed to several factors related to the surgical procedure and the healing process. After a clavicle fracture, especially when treated surgically with plates or screws, the shoulder joint may experience stiffness due to immobilization, inflammation, and the natural healing process of the surrounding tissues.
Understanding Shoulder Stiffness
1. Immobilization Effects: After surgery, the use of a sling or triangular bandage is often necessary to stabilize the shoulder and allow for proper healing. While this is crucial for recovery, prolonged immobilization can lead to stiffness in the shoulder joint and surrounding muscles. The muscles and tendons may become tight and less flexible due to lack of movement.
2. Inflammation and Healing: Post-surgical inflammation is a normal part of the healing process. This inflammation can lead to discomfort and stiffness as the body works to repair the damaged tissues. The presence of scar tissue can also contribute to a feeling of tightness or restriction in movement.
3. Muscle Weakness: Following surgery, the muscles around the shoulder may weaken due to disuse. This weakness can lead to a lack of support for the shoulder joint, making it feel unstable and stiff during movement.
4. Range of Motion: It is not uncommon for patients to experience a limited range of motion after removing the sling. The shoulder may feel "stuck" or "tight," especially when attempting to lift the arm or reach overhead. This can be frustrating, but it is often a temporary condition that improves with rehabilitation.
Is It Normal?
Yes, experiencing stiffness and a sensation of tightness in the shoulder after clavicle surgery is generally considered normal, especially in the early stages of recovery. However, it is essential to monitor the symptoms and ensure that they are gradually improving. If the stiffness persists or worsens, it may indicate a need for further evaluation.
Recommendations for Management
1. Physical Therapy: Engaging in a structured physical therapy program is one of the most effective ways to address shoulder stiffness. A physical therapist can provide exercises tailored to your specific needs, focusing on improving range of motion, strength, and flexibility. Gentle stretching and strengthening exercises can help alleviate stiffness and restore function.
2. Gradual Mobilization: It is crucial to gradually increase the range of motion of the shoulder. Start with gentle movements and progress to more challenging exercises as tolerated. Avoid pushing through severe pain, as this can lead to further complications.
3. Heat and Cold Therapy: Applying heat to the shoulder can help relax tight muscles and improve blood flow, while cold therapy can reduce inflammation and numb pain. Alternating between heat and cold can be beneficial.
4. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage discomfort associated with stiffness. Always consult with your physician before starting any medication.
5. Follow-Up with Your Surgeon: Regular follow-up appointments with your orthopedic surgeon are essential to monitor your recovery. If stiffness persists or if you experience new symptoms, such as increased pain or swelling, it is important to communicate these changes to your healthcare provider.
Conclusion
In summary, shoulder stiffness after clavicle surgery is a common occurrence and can be attributed to several factors, including immobilization, inflammation, and muscle weakness. While it is generally normal, it is essential to actively engage in rehabilitation and follow up with your healthcare provider to ensure a smooth recovery. With appropriate management and therapy, most patients can expect to regain full function and alleviate stiffness over time.
