Stiffness in the shoulder after fracture surgery?

Hello Doctor: I was in a car accident on November 22, 2019, and underwent clavicle surgery on December 2, 2019.
I have been returning for monthly check-ups and performing pendulum and up-and-down arm exercises daily, but I am hesitant to make large movements.
I have not raised my arm high or lifted heavy objects.
However, I still feel stiffness and a catching sensation in my shoulder, as if there is a string pulling between my clavicle and arm.
My condition has not improved significantly.
During my last follow-up appointment on January 29, 2020, the doctor reviewed my X-rays and mentioned that I am currently about 70% healed and advised further observation without recommending rehabilitation until I improve a bit more.
I have the following questions: 1.
Is it normal to experience stiffness and a catching sensation in my shoulder 2.5 months post-surgery, as well as the feeling of a string pulling between my clavicle and arm? Should I undergo further examinations, such as for nerve damage? 2.
Is it normal that I cannot raise my arm to 180 degrees or lift it high, and will I recover in the future? 3.
I've heard that the first three months post-surgery is a critical period for rehabilitation; if I cannot participate in rehabilitation, will there be any long-term consequences? Thank you for taking the time to respond, Doctor.

1.
This is a normal condition and does not require further examination.

2.
It is possible; after the bone heals, rehabilitation should be done gradually.

3.
The shoulder may become stiff and unable to return to its normal range of motion.

Shoulder stiffness after clavicle surgery can be a common concern for many patients, especially following trauma such as a car accident. In your case, it has been 2.5 months since your clavicle surgery, and you are experiencing persistent stiffness and a sensation of tightness between your clavicle and arm. Let's address your concerns systematically.

1. Is the stiffness and tightness normal? Should I seek further examination for potential nerve damage?
After clavicle surgery, it is not uncommon to experience stiffness and a feeling of tightness in the shoulder area. This can be attributed to several factors, including post-surgical inflammation, scar tissue formation, and muscle guarding due to pain. The sensation of a "pull" or tightness can also be related to the healing process of the surrounding soft tissues. However, if you are experiencing significant discomfort, weakness, or any unusual symptoms such as numbness or tingling in your arm or hand, it would be prudent to consult your physician for further evaluation. They may recommend imaging studies or a neurological assessment to rule out any nerve involvement.

2. Is it normal not to be able to raise my arm to 180 degrees, and will I recover full range of motion?
Limited range of motion, especially in the early stages of recovery, is quite common. After surgery, the shoulder joint may be stiff due to immobilization and the healing process. While you may not be able to lift your arm fully at this stage, many patients do regain their range of motion over time with appropriate rehabilitation. The timeline for recovery can vary significantly from person to person, depending on factors such as the extent of the injury, the surgical technique used, and individual healing rates. Engaging in gentle range-of-motion exercises, as advised by your healthcare provider, can help facilitate recovery.

3. Is it true that the first three months post-surgery is a critical period for rehabilitation, and what happens if I cannot participate in rehab?
The first three months after surgery are indeed considered a critical period for rehabilitation. During this time, the body is healing, and engaging in physical therapy can help optimize recovery and prevent stiffness. If rehabilitation is delayed or not initiated, there is a risk of developing long-term stiffness or decreased function in the shoulder. However, it is essential to follow your surgeon's recommendations regarding the timing of rehabilitation. If your doctor has advised waiting for further healing before starting formal rehabilitation, it is crucial to adhere to that guidance. Once you are cleared for physical therapy, a structured rehabilitation program can significantly enhance your recovery and help restore function.

In summary, while some stiffness and tightness are normal after clavicle surgery, it is essential to monitor your symptoms closely. If you have any concerns about nerve damage or if your symptoms worsen, do not hesitate to seek further evaluation. Engaging in rehabilitation when appropriate will be key to regaining your shoulder function. Always communicate openly with your healthcare team about your progress and any concerns you may have. They are there to support you through your recovery journey.
