Post-operative issues related to circumcision?
Hello, doctor.
I am a patient who had a circumcision 11 days ago.
A few days ago, the urethral opening had already healed, and the wound was almost completely healed with no bleeding.
However, I still experience a split stream when urinating.
Yesterday, during my follow-up appointment, the doctor said everything was fine.
But this afternoon, I suddenly felt unable to urinate, and when I finally did, it was extremely painful and there was bleeding.
It felt like my urethra was being torn open, and upon inspection, I saw that the wound had indeed reopened.
I tried to stay calm and immediately scheduled an appointment for tomorrow.
The bleeding has mostly stopped, and I applied Betadine.
However, I am unsure how to care for it moving forward, and I don't know if I should continue using saline solution before applying the ointment.
I hope you can clarify this for me.
Thank you very much!

Hello: The genital area can sometimes exhibit elasticity due to vascular engorgement, which makes it difficult to bandage and can hinder wound healing.
After bleeding, you can apply pressure to stop the bleeding by using a cotton swab for 5 minutes.
For larger wounds, apply pressure with sterile gauze or cotton.
Then, disinfect with saline solution and apply a thicker layer of ointment.
Try to minimize exposure to irritants or avoid friction to prevent engorgement or re-bleeding.
Wishing you good health.

Post-circumcision complications can be concerning, especially when it comes to managing pain and bleeding. It’s not uncommon for patients to experience some discomfort and complications after the procedure, particularly within the first few weeks. Based on your description, it seems you are experiencing a combination of pain, bleeding, and possibly some issues with urination, which can be distressing.
Firstly, it’s important to understand that after a circumcision, the area around the glans (the tip of the penis) can be sensitive and may take time to heal completely. The fact that you experienced pain and bleeding after urination suggests that there may be irritation or a small tear in the healing tissue. This can happen if there is friction or pressure on the area, especially during urination.
Here are some steps you can take to manage your symptoms and promote healing:
1. Keep the Area Clean: It’s crucial to maintain good hygiene to prevent infection. You can gently clean the area with mild soap and water. Avoid using harsh soaps or scrubbing the area too vigorously.
2. Use Saline Solution: If your doctor has recommended using saline solution, it can help to rinse the area gently. This can aid in keeping the wound clean and may help reduce irritation.
3. Apply Topical Treatments: If your doctor has prescribed or recommended a topical ointment (like a medicated cream or ointment), apply it as directed. Avoid using products that are not recommended by your healthcare provider, as they may cause further irritation.
4. Monitor for Signs of Infection: Keep an eye on the area for any signs of infection, such as increased redness, swelling, warmth, or discharge that has a foul odor. If you notice any of these symptoms, contact your healthcare provider immediately.
5. Pain Management: Over-the-counter pain relievers, such as acetaminophen or ibuprofen, can help manage pain. However, consult your doctor before taking any medication, especially if you have any underlying health conditions or are taking other medications.
6. Avoid Irritating Activities: Until the area has fully healed, avoid activities that may irritate the site, such as vigorous exercise or sexual activity. This will help prevent further injury and allow the area to heal properly.
7. Follow-Up with Your Doctor: Since you have already scheduled a follow-up appointment, make sure to discuss your symptoms in detail with your doctor. They may want to examine the area to ensure that there are no complications, such as a significant tear or infection.
8. Hydration and Diet: Staying well-hydrated can help with urination and may reduce discomfort. A balanced diet can also support your overall healing process.
In terms of your specific question about using saline solution followed by ointment, it’s generally advisable to clean the area first with saline and then apply any prescribed ointment. However, always follow your doctor’s specific instructions regarding post-operative care.
Lastly, while it’s normal to have some discomfort and complications after circumcision, persistent pain or bleeding should not be ignored. If your symptoms worsen or do not improve, seek medical attention promptly. Your healthcare provider is the best resource for personalized advice and treatment options based on your individual situation.
