I have some questions regarding chest tightness?

Hello Doctor: At the end of January, I experienced a scratchy throat and visited a clinic where I was diagnosed with post-nasal drip leading to sinusitis.
I have been taking antibiotics as prescribed, and my condition has improved, with only mild throat irritation remaining (but no cough).
I have also been taking traditional Chinese medicine for recovery.
Since February 11, I have not taken any medication.
However, starting February 14, I began to feel some tightness in my chest.
Breathing, pressing, or stretching does not cause pain, and lying flat provides some relief, but I feel tightness when standing.
Sometimes, I also experience fatigue and slight dizziness.
From February 1 to February 15, I was under significant stress due to work deadlines.
Should I consider visiting a doctor at the hospital for my current situation? Thank you for your assistance.

Dear MOMO: Hello.
Based on your description, it may be a case of autonomic nervous system dysfunction.
Since we are currently in a pandemic and to avoid going to the hospital, you can first adjust your daily routine.
If there is no improvement, please visit the hospital for further examination.

Chest tightness can be a concerning symptom, especially when it is accompanied by other signs such as fatigue and dizziness. Given your recent history of throat issues and the stress you mentioned from work, it is important to consider both physical and psychological factors that may contribute to your current symptoms.

Firstly, it is essential to differentiate between potential cardiac causes of chest tightness and other non-cardiac reasons. Cardiac issues, such as angina or myocardial ischemia, can present as chest tightness, especially under stress or physical exertion. In your case, the fact that the tightness improves when lying down but worsens when standing could suggest a non-cardiac origin, such as anxiety or muscular tension, particularly given your recent stress levels. However, it is crucial not to dismiss the possibility of a cardiac issue, especially since you have experienced fatigue and dizziness, which can also be associated with heart problems.

The symptoms you describe, including chest tightness that does not cause pain upon breathing or pressing, could indicate several conditions. These may range from anxiety-related chest tightness to musculoskeletal issues, such as costochondritis or muscle strain, especially if you have been under significant stress. However, it is also important to consider gastrointestinal issues, such as acid reflux, which can mimic cardiac symptoms.

Given your symptoms and the context of your recent health history, it is advisable to seek medical attention. A healthcare professional can perform a thorough evaluation, which may include a physical examination, an electrocardiogram (ECG), and possibly further imaging studies if deemed necessary. These assessments can help rule out any serious cardiac conditions and provide peace of mind.

In addition, if you are experiencing significant stress, it may be beneficial to discuss this with your doctor. Stress can manifest physically and may contribute to symptoms like chest tightness and fatigue. Addressing stress through lifestyle changes, relaxation techniques, or counseling may also be helpful.

In summary, while your symptoms might not immediately suggest a severe cardiac issue, it is always better to err on the side of caution. Seeking medical attention will ensure that any potential underlying conditions are identified and managed appropriately. Your health is paramount, and timely intervention can make a significant difference in outcomes.
