Chest discomfort?

Hello, doctor.
About a month ago, I suddenly felt a tightness in my chest as if my heart was being compressed, which lasted for about ten seconds.
I was quite worried, so I went to the hospital to see a cardiologist, but the doctor said there was nothing serious.
Since then, over the past month, I have experienced many new symptoms (some old symptoms seemed to disappear when new ones appeared).
Recently, I have been frequently feeling dizzy, and when I lie down, I experience discomfort in my heart.
When measuring my blood pressure, my diastolic pressure is often only in the low 50s to just over 60.
Occasionally, I have tightness around my collarbone.
Although I do not have lower limb edema, I am still very concerned about the possibility of heart failure or arrhythmia.
Should I consider seeing a pulmonologist?

It's understandable to feel anxious about chest discomfort, especially given the wide range of potential causes and the serious nature of heart-related issues. Your symptoms, including the sensation of tightness in your chest, dizziness, and low diastolic blood pressure, warrant careful consideration and further evaluation.
First and foremost, it's crucial to recognize that while many individuals experience occasional chest discomfort, persistent or recurrent symptoms should not be ignored. The fact that you have already consulted a cardiologist and were told there was no significant issue is reassuring, but it does not eliminate the need for ongoing vigilance, especially since you are experiencing new symptoms.

Chest discomfort can arise from various sources, including cardiac, gastrointestinal, musculoskeletal, and even psychological factors. Given your description of symptoms, such as the tightness in your chest and the sensation of discomfort when lying down, it may be beneficial to explore both cardiac and non-cardiac causes.
1. Cardiac Considerations: While your previous evaluation by a cardiologist was reassuring, if you are experiencing new symptoms like dizziness and low diastolic blood pressure, it would be prudent to follow up with your healthcare provider. They may recommend additional tests such as an echocardiogram, stress test, or Holter monitor to assess your heart's function and rhythm more thoroughly.
2. Non-Cardiac Causes: Conditions such as anxiety, panic attacks, or even gastrointestinal issues like acid reflux can manifest as chest discomfort. Anxiety, in particular, can lead to physical symptoms, including tightness in the chest, dizziness, and a feeling of breathlessness. If you suspect that anxiety may be contributing to your symptoms, discussing this with a mental health professional could be beneficial.

3. Blood Pressure Monitoring: You mentioned that your diastolic blood pressure readings are low. It's essential to monitor your blood pressure regularly and discuss these findings with your healthcare provider. Low blood pressure can lead to dizziness and may require further investigation.

4. Referral to a Specialist: If your symptoms persist or worsen, seeking a referral to a pulmonologist (chest specialist) may be appropriate, especially if you have concerns about lung-related issues contributing to your symptoms. They can evaluate for conditions such as asthma, chronic obstructive pulmonary disease (COPD), or other respiratory issues that might be causing your discomfort.

5. Lifestyle Factors: Consider lifestyle modifications that can help alleviate symptoms. Regular exercise, a balanced diet, adequate hydration, and stress management techniques can significantly impact your overall well-being.
In conclusion, while your initial evaluation by a cardiologist was reassuring, the emergence of new symptoms and ongoing concerns about your health warrant further investigation. It is always better to err on the side of caution when it comes to chest discomfort. Make an appointment with your healthcare provider to discuss your symptoms in detail and explore the next steps for evaluation and management. Remember, your health is paramount, and seeking help is a proactive step toward understanding and addressing your concerns.
