Questions Regarding Erection Issues

Hello Dr.
Lee, a few days ago I cleaned my smegma, possibly after not doing it for a long time.
I gradually rinsed it with water and used a sterilized cotton swab dipped in Betadine.
At the time, I felt a bit of irritation, but it subsided after a while, and there was no redness or swelling.
However, this morning I experienced some pain during morning erections, but there was no pain while urinating, and after urination, it wasn't as hard, and the pain went away.
I would like to ask if these symptoms are normal, or could it be that the pain is just due to pressure from my pants during sleep? I'm concerned that I might have injured myself while cleaning the smegma or that the foreskin hasn't returned to its normal position.
If I pulled it down, does that mean it has returned completely? I apologize for the lengthy question, and I appreciate your response.
Thank you.

Unless there is incarcerated phimosis, there should be no issue with the retraction of the foreskin.
As for the pain during erection, it may be due to minor injury during cleaning, which should not be a concern.
However, if there are worries, it is advisable to consult a urologist for an examination and further discussion with the physician.

Morning erections, commonly referred to as "morning wood," are a normal physiological occurrence for many men. They are typically a result of increased blood flow to the penis during REM (rapid eye movement) sleep, which is a stage of sleep associated with dreaming. However, experiencing pain during these erections can be concerning and may indicate an underlying issue.

From your description, it seems that you recently engaged in cleaning your foreskin, which may have caused some irritation or sensitivity. The initial discomfort you felt during cleaning could have been due to the friction or pressure applied while using the cotton swab and antiseptic. It’s important to be gentle when cleaning sensitive areas to avoid irritation or injury.

The pain you experienced during your morning erection could be attributed to several factors:
1. Irritation or Inflammation: If the cleaning process caused any minor irritation or inflammation of the skin, this could lead to discomfort during an erection. The penis can be sensitive, and any irritation can be exacerbated during an erection due to increased blood flow and pressure.

2. Pressure from Clothing: As you mentioned, wearing tight clothing while sleeping can also contribute to discomfort. If your pants were too tight, they could have restricted blood flow or caused pressure on the penis, leading to pain during an erection.

3. Normal Sensitivity: Sometimes, the body can be more sensitive in the morning due to various factors, including hormonal fluctuations and the state of relaxation during sleep. This can lead to temporary discomfort that resolves quickly, as you noted when the pain subsided after urination.

4. Potential Injury: If there was any trauma or injury to the area during your cleaning, it could lead to temporary pain. However, since you mentioned there was no visible redness or swelling, it seems less likely that there is a significant injury.

Regarding your concern about whether the foreskin has returned to its normal position, if you were able to pull it back and it returned to its original position without any issues, it is likely that it is in place. However, if you notice any persistent pain, swelling, or difficulty retracting the foreskin, it would be advisable to consult a healthcare professional.

In general, if the pain persists or worsens, or if you experience other symptoms such as swelling, redness, or discharge, it would be prudent to seek medical advice. A healthcare provider can perform a physical examination and provide a more accurate diagnosis and treatment options if necessary.

To prevent future discomfort, consider the following tips:
- Gentle Cleaning: When cleaning the area, use gentle motions and avoid harsh chemicals or excessive pressure. Warm water and mild soap are usually sufficient for cleaning.

- Comfortable Clothing: Ensure that your sleepwear is comfortable and not too tight, allowing for natural movement and blood flow.

- Monitor Symptoms: Keep track of any recurring pain or discomfort. If it happens consistently, it may indicate a need for further evaluation.

In conclusion, while occasional discomfort during morning erections can be normal, especially after cleaning or due to clothing pressure, persistent or severe pain should be evaluated by a healthcare professional. Taking care of your genital health is important, and addressing any concerns early can help prevent complications in the future.
