Post-operative issues after hemorrhoidectomy?
I had hemorrhoid surgery on January 17th, and my last follow-up appointment was on February 6th.
Since then, I have experienced occasional constipation, which has caused me to strain (sometimes resulting in bleeding).
Recently, while applying medication, I felt a protrusion inside the anus.
Could this be a recurrence of hemorrhoids, or is it a scar from the surgical site since the original hemorrhoids were quite large and the sutured wound is also larger?
Ms. Wu, 20~29 year old female. Ask Date: 2020/02/27
Dr. Ke Fangxu reply Surgery
Surgical removal of hemorrhoids does not treat constipation, so it is essential to avoid constipation postoperatively to prevent symptom recurrence.
Generally, recurrence does not happen quickly; it is likely due to congestion and swelling of the wound.

After undergoing a hemorrhoidectomy on January 17, you have expressed concerns regarding the possibility of a recurrence of hemorrhoids or the formation of scar tissue, especially since you have experienced constipation and occasional bleeding. It is understandable to feel anxious about these symptoms, particularly after a surgical procedure.
Firstly, it is important to recognize that the healing process after a hemorrhoidectomy can vary significantly from person to person. The surgical site may take several weeks to heal completely, and during this time, it is not uncommon to experience some discomfort, changes in bowel habits, and even the formation of scar tissue. Scar tissue can sometimes feel like a lump or bump, especially if the original hemorrhoid was large and required significant surgical intervention.
The presence of a lump or bump in the anal area post-surgery can be concerning. It could potentially indicate several things:
1. Scar Tissue Formation: After surgery, the body naturally forms scar tissue as part of the healing process. This scar tissue can sometimes create a palpable mass that may feel different from the surrounding tissue. This is generally not a cause for concern unless it is associated with significant pain or other symptoms.
2. Recurrence of Hemorrhoids: While hemorrhoids can recur after surgical removal, it is less common if the surgery was performed correctly and if post-operative care is followed. However, factors such as straining during bowel movements, constipation, and poor dietary habits can contribute to the development of new hemorrhoids. If you are experiencing constipation and are straining, it may increase the risk of recurrence.
3. Other Conditions: There are other conditions that could present with similar symptoms, such as anal fissures, skin tags, or even abscesses. If the lump is painful, red, or associated with significant bleeding, it is essential to seek medical attention.
Given your symptoms of occasional bleeding and the feeling of a lump, it is advisable to follow up with your surgeon or a gastroenterologist. They can perform a physical examination and possibly recommend further imaging or procedures if necessary. It is crucial to address any bleeding, as this can indicate complications or the need for further intervention.
In the meantime, managing your bowel habits is essential. Here are some recommendations to help prevent constipation and reduce the risk of straining:
- Increase Fiber Intake: Incorporate more fruits, vegetables, whole grains, and legumes into your diet. Fiber helps to soften stools and promote regular bowel movements.
- Stay Hydrated: Drink plenty of water throughout the day. Adequate hydration is vital for preventing constipation.
- Consider Stool Softeners: Over-the-counter stool softeners can help ease bowel movements and reduce the need to strain.
- Regular Exercise: Engaging in regular physical activity can help stimulate bowel function and prevent constipation.
- Avoid Straining: If you feel the urge to have a bowel movement, try to respond promptly to avoid straining.
In conclusion, while it is possible that the lump you are feeling is scar tissue, it is also important to rule out the recurrence of hemorrhoids or other conditions. A follow-up appointment with your healthcare provider will provide clarity and ensure that you receive the appropriate care. Remember, early intervention can prevent complications and promote a smoother recovery.
