Symptoms After a Head Injury: What to Expect - Neurosurgery

Brain issues

On the evening of February 25, 2020, after falling from a height of 130 cm while repairing the ceiling, the back of my head struck the concrete floor.
It has been 4 days since the incident, and the most painful point is when I press down behind my right ear.
If I gently touch down towards my jaw, it also hurts, and there is slight swelling in my jaw.
I experience some pain in my throat when eating or swallowing.
Could you please explain what areas might be injured? Why would hitting the back of my head on the concrete cause these symptoms? Will these symptoms subside over time? Thank you!

Aji, 60~69 year old female. Ask Date: 2020/02/29

Dr. Zhang Junwei reply Neurosurgery

In general, head and neck contusions typically heal within about a week.
If pain persists, it is recommended to schedule an appointment with a neurosurgery clinic for evaluation.

After a head injury, especially one involving a fall from a height, it's crucial to understand the potential symptoms and their implications. In your case, you experienced a fall from about 130 centimeters, resulting in a direct impact to the back of your head against a hard surface (concrete). This type of trauma can lead to various symptoms, some of which you are currently experiencing.

The pain you feel when pressing on the area behind your right ear and extending down towards your jaw could indicate a few things. First, it may suggest that you have sustained a contusion (a bruise) or a more serious injury to the soft tissues, muscles, or even the bones in that area. The swelling you mentioned around your jaw could be a result of localized inflammation due to the injury. Additionally, the pain when swallowing could indicate that the trauma has affected the muscles or nerves in the neck and throat area, leading to discomfort during these actions.

The symptoms you are experiencing are not uncommon following a head injury. When the head strikes a hard surface, the brain can move within the skull, potentially leading to a concussion or other forms of traumatic brain injury. Symptoms of a concussion can include headaches, dizziness, nausea, and sometimes difficulty with balance or coordination. In your case, the pain radiating from the injury site and the swelling could be a sign of a more localized injury rather than a concussion, but it is essential to monitor for any worsening symptoms.

As for whether these symptoms will resolve over time, many minor injuries to the head and neck can improve significantly within a week or two. However, if the pain persists or worsens, or if you develop new symptoms such as severe headaches, confusion, persistent vomiting, or changes in vision, it is crucial to seek medical attention promptly. These could be signs of more serious complications, such as intracranial bleeding or a fracture that may require further evaluation and treatment.

In general, after a head injury, it is advisable to rest and avoid activities that could exacerbate your condition, such as strenuous physical activity or tasks requiring significant concentration. Ice packs can help reduce swelling, and over-the-counter pain relievers may alleviate discomfort. However, be cautious with medications and consult with a healthcare provider if you have any concerns or if symptoms do not improve.

In summary, while some discomfort and swelling are expected after a head injury, it is essential to remain vigilant for any signs of complications. If you have any doubts about your recovery or if your symptoms change, do not hesitate to consult a healthcare professional for a thorough evaluation. Your health and safety should always come first, especially after experiencing a significant impact to the head.
