Post-hemorrhoid surgery
Hello Doctor, I would like to ask about my condition.
I had hemorrhoid surgery on December 20, and the severity of my hemorrhoids was classified as grade III.
It has been over two months since the surgery, and I have had a follow-up appointment.
The doctor performed a digital examination and said the wound looks good, but I often feel a sense of urgency to have a bowel movement.
Is this normal? I inquired about the need for a colonoscopy, but the doctor mentioned that since I just had one in April, it is generally unnecessary.
However, I experience this urgency, especially when lying on my side, which is more pronounced.
The doctor prescribed Loperamide 2mg, advising me to take it if the symptoms are significant, but I feel like it hasn't made much of a difference.
Could this be due to the surgery affecting my anal sphincter, making it more sensitive? What other possibilities could there be? Do you have any recommendations? Thank you, Doctor.

Hemorrhoid surgery generally does not damage the sphincter muscle, and it is common to experience slight abnormal sensations postoperatively.
Taking warm sitz baths or applying topical ointments can help alleviate symptoms.
A follow-up colonoscopy is indeed not necessary.
It is recommended to increase the intake of dietary fiber or take fiber supplements to shorten bowel movement time, which should also be beneficial.

After undergoing hemorrhoid surgery, particularly for a third-degree hemorrhoid, it is not uncommon to experience persistent sensations of urgency or sensitivity in the rectal area. This can be attributed to several factors related to the surgical procedure and the healing process.
Firstly, the surgery itself can lead to changes in the normal functioning of the anal sphincter and surrounding tissues. The surgical manipulation may cause temporary inflammation or irritation of the anal canal, which can manifest as a heightened sense of urgency or discomfort. This is especially true if the surgery involved excision of tissue or if there was significant swelling post-operation. The healing process can take time, and during this period, the nerves in the area may be more sensitive than usual.
Secondly, the sensation of urgency can also be linked to changes in bowel habits post-surgery. After hemorrhoid surgery, patients often experience changes in their bowel movements, including alterations in stool consistency and frequency. If the stool is softer or more frequent due to dietary changes or medications, this can lead to a feeling of urgency. The prescription of Loperamide, an anti-diarrheal medication, suggests that your doctor is considering the possibility of loose stools contributing to your symptoms. However, if you feel that Loperamide is not providing relief, it may be worth discussing alternative management strategies with your physician.
Another consideration is the psychological aspect of surgery. After undergoing a procedure, some patients may develop heightened awareness of their bodily sensations, leading to an increased perception of urgency. This can be exacerbated by anxiety or stress related to the surgery and recovery process.
Regarding the possibility of damage to the anal sphincter during surgery, while it is a rare complication, it can occur. If the sphincter is affected, it may lead to issues with bowel control or increased sensitivity. If you have concerns about this, it would be prudent to discuss them with your surgeon or a gastroenterologist. They may recommend further evaluation, such as an anal manometry test, to assess sphincter function.
As for the need for a colonoscopy, your doctor’s recommendation to wait until April, given that you had a recent examination, is reasonable. However, if your symptoms persist or worsen, it is essential to communicate this to your healthcare provider. They may decide that further investigation is warranted to rule out any underlying issues, such as inflammation or other gastrointestinal conditions.
In summary, while persistent urgency and sensitivity after hemorrhoid surgery can be concerning, they are not uncommon. Factors such as surgical trauma, changes in bowel habits, and psychological responses can all contribute to these sensations. It is crucial to maintain open communication with your healthcare provider, report any changes in your symptoms, and discuss any concerns you may have regarding your recovery. If symptoms persist or significantly impact your quality of life, further evaluation may be necessary to ensure appropriate management and care.
