Inquiry about gum issues?
Hello Dr.
Tseng, a few days ago while brushing my teeth, I accidentally brushed too hard and started bleeding.
After rinsing, I felt a bit of a stinging pain.
This morning, when I looked at the bleeding part of my gums, I noticed there was some white tissue underneath my teeth.
However, the upper teeth do not hurt at all.
I would like to know if it is normal to see white tissue that looks like bone underneath the gums of the upper or lower teeth.
I'm not sure if this could be something abnormal or just the tooth roots, as I haven't paid much attention to it before.
Additionally, is it okay to apply oral ointment on the injured area, or should I just let it heal on its own?

Hello, Bobo: Brushing too hard can cause bleeding gums and white patches, which may indicate gum injury leading to ulceration.
If there is pain and inflammation, you can apply an oral ointment, but if you don't apply anything, just maintaining good oral hygiene should help it heal gradually in about a week.

Understanding gum issues, especially when they involve symptoms like bleeding, pain, and a white appearance, can be quite concerning. Based on your description, it seems that you may be experiencing a common issue related to gum health, possibly exacerbated by aggressive brushing.
When you brush your teeth too hard, it can lead to trauma to the gums, resulting in bleeding and discomfort. The white appearance you are noticing could be due to several factors. One possibility is that it is a sign of gingivitis, which is an inflammation of the gums often caused by plaque buildup. In some cases, the white appearance could also be related to the presence of necrotic tissue or a condition known as leukoplakia, which can occur in response to irritation or inflammation. However, leukoplakia is more commonly associated with tobacco use and other irritants.
It is important to note that the visibility of the tooth roots, which may appear white, is generally normal, especially if the gums have receded. The roots of the teeth are covered by cementum, which can sometimes be visible if the gums are not healthy. If you have not experienced pain in the upper teeth and the lower teeth are the only ones affected, it may indicate localized gum issues rather than a systemic problem.
Regarding the use of oral ointments or topical treatments on the affected area, it is generally advisable to avoid applying anything directly to open wounds unless recommended by a healthcare professional. Instead, maintaining good oral hygiene is crucial. This includes gentle brushing with a soft-bristled toothbrush, using fluoride toothpaste, and possibly rinsing with an antiseptic mouthwash to help reduce bacteria in the mouth.
If the bleeding and pain persist beyond a few days, or if you notice any changes in the appearance of your gums or teeth, it is essential to return to your dentist for a thorough examination. They can provide a more accurate diagnosis and recommend appropriate treatment options.
In the meantime, here are some additional tips to help manage your gum health:
1. Gentle Brushing: Use a soft-bristled toothbrush and brush gently to avoid further irritation to your gums.
2. Flossing: Incorporate daily flossing to remove plaque and food particles from between your teeth, which can help prevent gum disease.
3. Hydration: Stay hydrated and consider using a saline rinse (a mixture of salt and warm water) to help soothe your gums.
4. Diet: Maintain a balanced diet rich in vitamins and minerals, particularly vitamin C, which is essential for gum health.
5. Avoid Irritants: If you smoke or use tobacco products, consider quitting, as these can significantly impact gum health.
6. Regular Dental Check-ups: Schedule regular visits to your dentist for cleanings and check-ups to monitor your gum health and address any issues early.
In summary, while some symptoms you are experiencing may be common and manageable, it is crucial to monitor your condition closely. If symptoms worsen or do not improve, seeking professional dental advice is the best course of action to ensure your oral health is maintained.
