Chest tightness?

Hello Doctor, over the past year, I have experienced this sensation about 4 to 5 times.
It's hard to describe, but it feels somewhat like my heart suddenly tightening or being cut by a knife (I know this description sounds strange).
It lasts for about 1 to 2 seconds.
I have visited a large hospital to see a cardiologist and also consulted with a traditional Chinese medicine practitioner, and both said there is no problem, so I should not worry.
However, I have been a bit anxious and have been researching this issue, and I found this platform for inquiries.
Thank you very much.
I have some anxiety issues and occasionally experience palpitations.
I also have a mild mitral valve issue (the doctor mentioned that many people have this).
I would like to ask Dr.
He if this sensation is not a major concern.
I am feeling a bit scared.
Thank you.

Dr. He Dongjin reply Cardiology

Hello, it seems that the autonomic nervous system disorder is caused by a mitral valve issue.
Since the examination at the major hospital showed no problems, the doctor said there's no need to worry.
Changhua Hospital cares about you.

Understanding heart tightness can be a complex issue, especially when it is accompanied by anxiety and other symptoms. Based on your description, it seems that you have experienced episodes of heart tightness that last for a brief period, accompanied by feelings of anxiety and a known history of mild mitral valve prolapse. While it is reassuring that you have consulted with healthcare professionals who have ruled out significant heart issues, it is understandable to still feel concerned about these sensations.

Heart tightness or discomfort can be caused by a variety of factors, and it is essential to consider both physical and psychological aspects. In your case, the episodes you describe—lasting 1 to 2 seconds and feeling like a sudden tightening or sharp pain—could be related to several potential causes:
1. Anxiety and Panic Attacks: Given your history of anxiety, it is possible that these episodes are related to anxiety or panic attacks. Anxiety can manifest in physical symptoms, including heart palpitations, tightness in the chest, and a feeling of impending doom. During periods of heightened stress or anxiety, the body can react with a "fight or flight" response, leading to increased heart rate and muscle tension, which may contribute to the sensations you are experiencing.

2. Mitral Valve Prolapse: You mentioned having a mild mitral valve issue, which is relatively common and often benign. In some cases, mitral valve prolapse can lead to palpitations or atypical chest sensations. While it is generally not a cause for concern, it is essential to monitor any changes in symptoms and discuss them with your healthcare provider.

3. Musculoskeletal Issues: Sometimes, tightness in the chest can be related to musculoskeletal problems, such as muscle strain or tension in the chest wall. Stress and anxiety can lead to muscle tension, which may contribute to the sensations you are feeling.

4. Gastroesophageal Reflux Disease (GERD): If you have a history of gastrointestinal issues, such as acid reflux, it can also cause sensations that mimic heart tightness. Acid reflux can lead to discomfort in the chest area, which may be mistaken for heart-related symptoms.

5. Cardiac Issues: While you have been evaluated and cleared of significant heart problems, it is always wise to remain vigilant. If you experience new or worsening symptoms, such as prolonged chest pain, shortness of breath, or dizziness, it is crucial to seek medical attention promptly.

To address your concerns and manage your symptoms, consider the following strategies:
- Stress Management: Engage in relaxation techniques such as deep breathing exercises, meditation, or yoga. These practices can help reduce anxiety and improve your overall sense of well-being.

- Regular Exercise: Physical activity can be an effective way to manage anxiety and improve cardiovascular health. Aim for at least 150 minutes of moderate-intensity exercise each week, as recommended by health guidelines.

- Healthy Lifestyle Choices: Maintain a balanced diet, stay hydrated, and avoid excessive caffeine and alcohol, which can exacerbate anxiety and palpitations.

- Cognitive Behavioral Therapy (CBT): If anxiety is significantly impacting your life, consider seeking therapy. CBT is an effective treatment for anxiety disorders and can help you develop coping strategies.

- Follow-Up with Healthcare Providers: Continue to communicate with your healthcare providers about your symptoms. If you feel that your anxiety is not being adequately addressed, consider discussing this with a mental health professional.

In conclusion, while the episodes of heart tightness you are experiencing can be distressing, they are likely related to anxiety and other benign factors, especially given the evaluations you've undergone. However, it is essential to remain proactive about your health and seek further evaluation if your symptoms change or worsen. Remember, you are not alone in this, and there are effective treatments available to help you manage both your physical and emotional health.
