Persistent urge to urinate in the penis?
Hello doctor, after I use the restroom, I still feel a persistent urge to urinate.
I do not experience nocturia, and upon waking, I no longer have that urge.
However, after my first morning urination, I still feel a sense of urgency in my penis.
I drink at least 2000 ml of water daily.
What could this issue be?

Dr. Li Xinyi reply Urology
It is possible that there is overactive bladder syndrome, or there may be an undetected urinary tract infection that has not been properly treated.
It is recommended to see a urologist for an examination and to discuss further with the physician.

The persistent urge to urinate after bathroom visits can be a frustrating and uncomfortable experience. This condition, often referred to as "post-void dribbling" or "urinary urgency," can have several underlying causes. Understanding these causes is essential for determining the appropriate course of action.
One common reason for experiencing a persistent urge to urinate is bladder overactivity. This condition occurs when the bladder muscles contract involuntarily, leading to a frequent and urgent need to urinate, even when the bladder is not full. Factors that can contribute to bladder overactivity include neurological conditions, bladder infections, or even certain medications that affect bladder function.
Another potential cause could be urinary tract infections (UTIs). UTIs can irritate the bladder and urethra, leading to a sensation of urgency and discomfort. If you have experienced any additional symptoms such as burning during urination, cloudy or foul-smelling urine, or pelvic pain, it would be wise to consult a healthcare provider for a proper evaluation.
Prostatitis, or inflammation of the prostate gland, is another condition that can lead to similar symptoms in men. This condition can cause pain and discomfort in the pelvic area, along with urinary urgency. If you are experiencing pain in the lower abdomen or pelvic region, it is important to discuss this with your doctor.
Additionally, benign prostatic hyperplasia (BPH), a non-cancerous enlargement of the prostate gland, can also lead to urinary urgency and frequency. This condition is common in older men and can cause a variety of urinary symptoms, including difficulty starting urination, weak urine flow, and the sensation of incomplete bladder emptying.
In some cases, psychological factors such as anxiety or stress can contribute to urinary urgency. The mind-body connection is powerful, and feelings of anxiety can lead to increased awareness of bodily sensations, including the urge to urinate.
Given that you mentioned drinking around 2000 ml of water daily, it is also important to consider whether your fluid intake is appropriate for your individual needs. While staying hydrated is crucial, excessive fluid intake can lead to increased urinary frequency. Monitoring your fluid intake and adjusting it based on your activity level and environmental conditions may help alleviate some of the urgency.
It is advisable to keep a diary of your urinary habits, including the frequency of urination, the volume of urine produced, and any associated symptoms. This information can be valuable for your healthcare provider in diagnosing the underlying cause of your symptoms.
In conclusion, persistent urge to urinate after bathroom visits can stem from various causes, including bladder overactivity, urinary tract infections, prostatitis, benign prostatic hyperplasia, or psychological factors. It is essential to consult with a healthcare provider for a thorough evaluation and appropriate management. They may recommend tests such as urinalysis, bladder scans, or even referrals to specialists if necessary. Addressing this issue promptly can help improve your quality of life and alleviate any discomfort you may be experiencing.
