The glans penis is sometimes red?

The glans sometimes appears red, and upon examination during bathing, there are small red spots or patches that are not painful or itchy, and there are no raised areas.
Is this normal? Could it be caused by changes in weather or by the foreskin being constricted? (I have phimosis but no paraphimosis, and I have no sexual experience; I clean it daily.) If the foreskin does not retract during an erection, would you recommend circumcision?

Mr. He, 10~19 year old female. Ask Date: 2020/04/07

Dr. Liu Mingyue reply Urology

Hello: It is often caused by fungi, damp heat, and lack of cleanliness.
Wash with clean water, dry thoroughly, and wear loose-fitting underwear.
Even after improvement, it is important to remain vigilant as it may recur.
If the foreskin cannot retract during an erection, circumcision is recommended by the physician; however, some individuals choose not to undergo surgery.
The decision is yours.
Wishing you good health.

The redness on the glans (the head of the penis) can be concerning, especially when it appears intermittently. However, in your case, since you mentioned that it is not accompanied by pain, itching, or any raised lesions, it is likely not a serious issue. Here are some potential causes and considerations regarding the redness you are experiencing:
1. Normal Variations: The glans can exhibit variations in color due to several benign factors. The skin on the glans is sensitive and can change color based on various stimuli, including temperature, friction, or even mild irritation from soaps or detergents. If the redness appears after washing or during certain activities, it may simply be a reaction to these factors.

2. Hygiene and Irritation: Since you mentioned that you have a longer foreskin but do not have phimosis (the inability to retract the foreskin), it's essential to maintain good hygiene. Regularly cleaning the area can help prevent irritation and infections. However, if the foreskin is too tight or if there is excessive moisture trapped under the foreskin, it can lead to irritation and redness.
3. Weather Changes: Environmental factors, such as temperature and humidity, can also affect the skin. For instance, increased sweating in warmer weather or dryness in colder conditions can lead to temporary changes in the appearance of the skin, including redness.

4. Potential Conditions: While the absence of pain or itching is reassuring, persistent redness could sometimes indicate conditions such as balanitis (inflammation of the glans), which can be caused by infections, irritants, or allergic reactions. If the redness persists or worsens, it would be advisable to consult a healthcare professional for a thorough examination.

5. Circumcision Consideration: Regarding your question about circumcision, if you experience difficulties with the foreskin during erections (such as it not retracting), it may be worth discussing with a urologist. Circumcision can alleviate issues related to tight foreskin and may also reduce the risk of infections. However, this decision should be made after considering all options and discussing them with a healthcare provider.

6. Monitoring Changes: Keep an eye on the redness. If it becomes persistent, changes in appearance, or is accompanied by other symptoms (such as discharge, pain, or swelling), it is crucial to seek medical advice.
In summary, while occasional redness of the glans can be normal, especially in the context of hygiene practices and environmental factors, it is essential to monitor the situation. If you have concerns about your foreskin or any changes in your genital health, consulting a healthcare professional is the best course of action. They can provide personalized advice and treatment options based on a thorough evaluation of your condition.
