Throat issues

Since before the Lunar New Year, I have been experiencing discomfort in the right side of my throat, and multiple visits to clinics have not resolved the issue.
I have also been to Taipei Veterans General Hospital three times, but the situation remains the same.
The symptoms are as follows: the discomfort is located just below the right thyroid cartilage, and it varies daily.
Sometimes it feels swollen and painful, with a sensation of a foreign body when swallowing; after 1-2 days, it turns into a sharp pain or a burning sensation in the throat, but the foreign body sensation disappears.
After another 3-4 days, it returns to the swollen and painful feeling with the foreign body sensation.
This cycle continues repeatedly.
During my first visit to the hospital, the doctor suggested it was gastroesophageal reflux disease (GERD) and prescribed medications for the stomach, expectorants, and pain relievers, which were ineffective.
On the second and third visits, I was given pain relievers and told to continue monitoring the situation.

Hello: Chronic pharyngitis is common and often related to lifestyle habits.
In addition to medication, adjustments in daily routines are necessary for complete improvement, such as reducing coffee, tea, and desserts, as well as minimizing spicy foods to alleviate gastroesophageal reflux.
If allergic rhinitis is present, medication or nasal sprays should be used for control.
If smoking is a factor, quitting smoking is essential.
Wishing you peace and health.
Sincerely, Dr.
Jue-Yi Cheng, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Persistent throat discomfort can be a frustrating and distressing experience, especially when it seems to have no clear cause or resolution. Based on your description, it appears that you have been experiencing a cycle of symptoms that include swelling, pain, and a sensation of something being stuck in your throat, which can vary in intensity and nature over time. This kind of discomfort can be attributed to several potential issues, and understanding these can help guide you toward effective treatment.

Possible Causes of Throat Discomfort
1. Gastroesophageal Reflux Disease (GERD): As your doctor suggested, GERD is a common cause of throat discomfort. It occurs when stomach acid flows back into the esophagus, leading to irritation. Symptoms can include a burning sensation in the throat, a feeling of lumpiness, and pain during swallowing. If the medications prescribed (like proton pump inhibitors) did not alleviate your symptoms, it may be worth discussing further diagnostic options or lifestyle changes that could help manage GERD.

2. Postnasal Drip: This condition occurs when excess mucus accumulates in the back of the throat, often due to allergies or sinus infections. This can lead to throat irritation, coughing, and a sensation of something stuck in the throat. Treatment may involve antihistamines or nasal sprays to reduce mucus production.

3. Throat Infections: Both viral and bacterial infections can cause throat discomfort. Conditions like pharyngitis or tonsillitis can lead to pain and swelling. If your symptoms persist, a throat culture or rapid strep test may be warranted to rule out infections.

4. Muscle Tension or Stress: Sometimes, throat discomfort can be related to muscle tension, especially if you are experiencing stress or anxiety. This can lead to a sensation of tightness or discomfort in the throat area. Relaxation techniques, speech therapy, or physical therapy may be beneficial in these cases.

5. Allergies: Allergic reactions to environmental factors (like pollen, dust, or pet dander) can cause throat irritation. Identifying and avoiding allergens, along with using antihistamines, may help alleviate symptoms.

6. Throat Cancer: Although less common, persistent throat discomfort can sometimes be a sign of more serious conditions, including throat cancer. If you have risk factors (such as smoking or heavy alcohol use) or if your symptoms persist despite treatment, further evaluation by an ENT specialist may be necessary.

Recommended Next Steps
1. Follow-Up with an ENT Specialist: Since you have already seen doctors at Taipei Veterans General Hospital, consider asking for a referral to an ENT specialist if you haven't already seen one. They may perform a laryngoscopy to visually examine your throat and identify any abnormalities.

2. Keep a Symptom Diary: Documenting when your symptoms occur, their intensity, and any associated factors (like meals, activities, or stress levels) can provide valuable information to your healthcare provider.

3. Consider Lifestyle Modifications: If GERD is a concern, lifestyle changes such as avoiding spicy foods, eating smaller meals, and not lying down immediately after eating can be beneficial. Elevating the head of your bed may also help reduce nighttime symptoms.

4. Explore Stress Management Techniques: If stress or anxiety is contributing to your symptoms, consider exploring relaxation techniques such as deep breathing, meditation, or yoga.

5. Seek a Second Opinion: If your symptoms persist and you feel that your concerns are not being adequately addressed, seeking a second opinion from another ENT specialist may provide new insights or treatment options.

Conclusion
Persistent throat discomfort can stem from various causes, and it often requires a multifaceted approach to diagnosis and treatment. By working closely with healthcare professionals, keeping track of your symptoms, and considering lifestyle adjustments, you can take proactive steps toward finding relief. Remember, it’s essential to communicate openly with your healthcare providers about your symptoms and any concerns you may have, as this will help them tailor the best treatment plan for you.
