Sudden bleeding 18 days after a medical abortion?

Hello doctor, I discovered I was pregnant for 4 to 5 weeks at the end of March and decided to undergo a medical abortion.
After taking the medication, I experienced bleeding for over a week, which then stopped.
However, in the third week, specifically on the 18th day, I started bleeding again.
Initially, the blood was darker, but later it turned bright red.
I would like to know what could be causing this bleeding, as it had already stopped for about a week and then suddenly resumed with a volume similar to that of a menstrual period.

Mi Mo, 20~29 year old female. Ask Date: 2020/04/14

Experiencing unexpected bleeding after a medical abortion can be concerning, especially when it occurs several weeks after the procedure. In your case, you mentioned that you had a medical abortion at around 4-5 weeks of pregnancy, and after an initial period of bleeding that lasted for over a week, you experienced a return of bleeding on the 18th day post-abortion. This bleeding initially presented as dark red and later turned bright red, resembling menstrual flow.

There are several potential causes for this unexpected bleeding:
1. Incomplete Abortion: One of the most common reasons for post-abortion bleeding is that not all of the pregnancy tissue has been expelled from the uterus. This can lead to ongoing bleeding as the body attempts to clear the remaining tissue. An ultrasound may be necessary to confirm whether the uterus is clear.

2. Hormonal Changes: After a medical abortion, your hormone levels, particularly human chorionic gonadotropin (hCG), will decrease. Fluctuations in hormone levels can lead to irregular bleeding or spotting as your body adjusts.
3. Uterine Contractions: The uterus may contract as it returns to its pre-pregnancy state, which can sometimes cause bleeding. This is a normal part of the recovery process, but if the bleeding is heavy or accompanied by severe pain, it warrants further investigation.

4. Infection: Although less common, an infection in the uterus can cause abnormal bleeding. Symptoms of infection may include fever, foul-smelling discharge, or severe abdominal pain. If you experience these symptoms, it is crucial to seek medical attention promptly.

5. Menstrual Cycle Resumption: After a medical abortion, your menstrual cycle may take some time to normalize. The bleeding you are experiencing could be an early return of your menstrual period, especially if it resembles your typical menstrual flow. However, this is usually not expected to occur so soon after the procedure.

Given your situation, here are some recommendations:
- Monitor the Bleeding: Keep track of the amount and color of the blood. If the bleeding becomes heavier than a typical menstrual period, or if you experience severe pain, you should seek medical attention immediately.

- Follow-Up Appointment: It is advisable to schedule a follow-up appointment with your healthcare provider. They may perform an ultrasound to ensure that your uterus is clear and to rule out any complications.

- Avoid Self-Diagnosis: While it is natural to seek answers, avoid jumping to conclusions about the cause of your bleeding without professional evaluation. Each case is unique, and only a healthcare provider can give you a definitive diagnosis.

- Rest and Care: Ensure you are taking care of yourself during this time. Rest, stay hydrated, and avoid strenuous activities until you have been evaluated by a healthcare professional.

In conclusion, while some bleeding after a medical abortion can be normal, the sudden return of bleeding after a period of cessation should be evaluated by a healthcare provider to rule out any complications. Your health and well-being are paramount, and timely medical advice can help address any concerns you may have.
