Concerns about ejaculation
Hello, Doctor.
I am the one who brought up cases 153843 and 154399.
After following your advice to reduce the frequency, I have indeed noticed a decrease in the unusual sensation after ejaculation.
I can feel the last few weak contractions, and that’s when the sensation occurs, but it disappears immediately afterward.
However, one day I ejaculated more than once, and since the volume of the second ejaculation was smaller, it ended more quickly, and I didn’t experience that sensation at all.
I am curious about what you mentioned last time regarding a urethral issue.
Could it be related to the pressure exerted on the urethra during contractions? I wonder if this issue is normal and whether I need any treatment or if it will resolve on its own.
P.S.
I want to mention that in the past, I didn’t seem to feel anything, but since I tried to clean my penis with soap two years ago, it caused some itching that persisted.
I went to see a doctor, who tested my urine and said there was no infection, only that my foreskin was a bit tight.
The doctor prescribed a steroid cream, but I forgot to ask how long to apply it, so I ended up using it for over a month, which made me feel uncomfortable.
After improper handling, it became inflamed, and I was very anxious at that time.
After it healed, I started to notice this issue while masturbating.
I feel like I have a lot of hair.
Thank you again for your help, Doctor.

Hello: The ejaculation outlet is located at the distal end of the urethra, in the perineal area.
Excessive ejaculation can have an impact.
Wishing you good health.

Understanding ejaculation concerns can be complex, and it's great that you're seeking insights from urology experts. Based on your description, it seems you're experiencing some unusual sensations during and after ejaculation, which can be influenced by various factors, including anatomical, physiological, and psychological aspects.
Firstly, the sensations you feel during ejaculation, particularly the contractions and the subsequent feelings, are typically related to the muscular contractions of the pelvic floor and the urethra. These contractions are essential for the ejaculation process, as they help propel semen out of the penis. The fact that you noticed a difference in sensation after reducing the frequency of ejaculation suggests that there may be a connection between the frequency of sexual activity and the sensitivity of your urethra and surrounding tissues.
Regarding your concern about whether this is normal, it is not uncommon for individuals to experience variations in sensation during ejaculation. Factors such as anxiety, frequency of sexual activity, and even the state of your physical health can all play a role. If you find that the sensations are bothersome or if they change significantly, it may be worth discussing this further with a urologist.
You mentioned that you had some issues with tight foreskin and discomfort after using soap, which can lead to irritation or inflammation. It's important to be cautious with hygiene practices, especially in sensitive areas. Using mild, unscented soap and avoiding excessive scrubbing can help prevent irritation. If you have tight foreskin (phimosis), this can also contribute to discomfort during sexual activity or ejaculation. In some cases, a urologist may recommend treatments ranging from topical steroids to surgical options, depending on the severity of the condition.
As for the treatment options, if the sensations you are experiencing are not causing significant distress and do not interfere with your sexual function, it may be reasonable to monitor the situation. However, if you feel that the sensations are impacting your quality of life or sexual satisfaction, it would be advisable to consult with a urologist. They can perform a thorough examination and may suggest treatments such as pelvic floor physical therapy, medications, or other interventions tailored to your specific needs.
In summary, while some variations in sensation during ejaculation can be normal, it's essential to listen to your body and seek medical advice if you have concerns. Maintaining good hygiene, managing anxiety, and discussing any ongoing issues with a healthcare professional can help you navigate these concerns effectively. Remember, open communication with your healthcare provider is key to addressing any urological issues you may face.
