Suspected premature ejaculation?
Hello Doctor: I would like to ask about my situation.
During foreplay (oral sex) for about 5 minutes, I experience a feeling of ejaculation, and when I stop, I find it easy to lose my erection.
Most of the time, after foreplay, when I enter the vagina, I ejaculate in about 1 minute.
Are there any methods to improve this? Is this condition considered premature ejaculation? I exercise regularly and masturbate about 1-2 times a week.

More Info
Premature ejaculation (PE) is a common sexual dysfunction characterized by the inability to control ejaculation during sexual activity, leading to ejaculation occurring sooner than desired. This condition can be distressing for both partners and can impact sexual satisfaction. Understanding the causes and potential solutions is crucial for those experiencing this issue.
Causes of Premature Ejaculation
1. Psychological Factors: Anxiety, stress, and relationship issues can significantly contribute to PE. Performance anxiety, in particular, can create a cycle where the fear of premature ejaculation leads to increased anxiety, which in turn exacerbates the problem.
2. Biological Factors: Certain biological factors may also play a role. These can include hormonal imbalances, neurotransmitter levels, and sensitivity of the penis. Some studies suggest that men with higher levels of serotonin may experience delayed ejaculation, while those with lower levels may ejaculate more quickly.
3. Medical Conditions: Conditions such as prostatitis (inflammation of the prostate), thyroid problems, or other health issues can also lead to premature ejaculation. It’s essential to rule out any underlying medical conditions that might be contributing to the problem.
4. Lifestyle Factors: Factors such as excessive alcohol consumption, drug use, and lack of physical fitness can also influence sexual performance. Regular exercise and a healthy lifestyle can improve overall sexual health.
Solutions and Management Strategies
1. Behavioral Techniques:
- Start-Stop Technique: This involves stimulating the penis until you feel close to ejaculation, then stopping until the urge decreases. This can help you gain better control over your ejaculation.
- Squeeze Technique: Similar to the start-stop method, this involves squeezing the base of the penis when you feel close to ejaculation to reduce the urge.
2. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels can help improve control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles used to stop urination.
3. Desensitizing Products: Over-the-counter creams or sprays that contain numbing agents can help reduce sensitivity and delay ejaculation. However, it’s important to use these products as directed and to consult with a healthcare provider if unsure.
4. Counseling and Therapy: If psychological factors are contributing to PE, speaking with a therapist or counselor can be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and improve sexual confidence.
5. Medication: In some cases, medications may be prescribed to help manage premature ejaculation. Selective serotonin reuptake inhibitors (SSRIs), commonly used as antidepressants, have been found to delay ejaculation as a side effect. Other medications may also be considered based on individual needs.
6. Communication with Partner: Open communication with your partner about your experiences and feelings can help reduce anxiety and improve intimacy. Discussing preferences and exploring different sexual activities can also enhance sexual satisfaction.
Conclusion
If you are experiencing symptoms of premature ejaculation, it is advisable to consult with a healthcare professional for a thorough evaluation. They can help determine the underlying causes and recommend appropriate treatment options tailored to your situation. Remember, PE is a common issue, and with the right strategies and support, it can be effectively managed.
