Delayed ovulation?

Hello Doctor: My menstrual cycle has always been regular, but this time it is a week late.
I am very worried that I might be pregnant.
The last time I had intercourse was on April 10th.
I took two pregnancy tests, both showing one line, but I still feel uneasy, so I went to see a doctor.
The doctor performed an ultrasound and said that I am currently in my ovulation period, and my endometrial thickness is 1.3 cm.
The doctor also asked me to take another pregnancy test, which again showed one line.
The doctor advised me to wait two weeks to see if my period does not come; if it doesn't, I should take a pregnancy test.
For the past two days, I have been experiencing slight discomfort in my lower abdomen and some breast tenderness, but it comes and goes.
I would like to ask the doctor, is it possible that I am pregnant?

Elaine, 10~19 year old female. Ask Date: 2020/04/20

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, the chances of pregnancy should be low.
Let's wait and see.
Wishing you good health.

When it comes to delayed menstruation, many women often wonder if it could indicate pregnancy. In your case, you mentioned that your period is usually regular, but this time it has been delayed by a week. You also noted that you had unprotected intercourse on April 10 and have taken two pregnancy tests, both of which returned negative results. Additionally, you consulted a doctor who performed an ultrasound and indicated that you are currently in your ovulation phase, with a uterine lining measuring 1.3 cm.
First and foremost, it is important to understand that a missed period can be caused by various factors, and pregnancy is just one of them. Stress, hormonal imbalances, changes in weight, illness, and lifestyle changes can all contribute to menstrual irregularities. Given that your doctor has confirmed that you are in your ovulation phase, it is possible that your body is simply experiencing a delay in menstruation due to these factors.

The fact that you have taken two pregnancy tests and both have shown a single line (negative) is a strong indicator that you are not pregnant. Home pregnancy tests are generally reliable when used correctly, especially if taken after a missed period. However, if you are testing too early, it may not detect the pregnancy hormone (hCG) if it is not yet at detectable levels.
Your doctor’s advice to wait two weeks before testing again is sound. If your period does not arrive by then, it would be prudent to take another pregnancy test. The ultrasound showing that you are in your ovulation phase suggests that your body is functioning normally in terms of ovulation, which is a positive sign for your reproductive health.

As for the mild discomfort in your lower abdomen and slight breast tenderness, these symptoms can be associated with ovulation or the onset of your menstrual cycle. It is not uncommon for women to experience premenstrual symptoms, which can sometimes mimic early pregnancy symptoms.
In summary, while the possibility of pregnancy cannot be entirely ruled out, the combination of negative pregnancy tests, the ultrasound findings, and your current symptoms suggest that it is more likely that your period is simply delayed due to non-pregnancy-related factors. If your period does not arrive within the next couple of weeks, or if your symptoms worsen, it would be advisable to follow up with your healthcare provider for further evaluation. They may consider additional tests or assessments to determine the underlying cause of your delayed menstruation.
In the meantime, try to manage stress and maintain a healthy lifestyle, as these factors can significantly impact your menstrual cycle. Remember that it is always best to consult with your healthcare provider for personalized advice and guidance based on your specific situation.
