Treatment options for OPLL (Ossification of the Posterior Longitudinal Ligament) and estimated costs?

Hello Doctor, I was recently diagnosed with OPLL at the cervical spine levels C3-C7.
The doctor suggested performing laminoplasty with a mini plate through a posterior approach, or an anterior cervical disc replacement surgery.
I am currently contemplating the cost issues.
Could you please advise if minimally invasive surgery for laminoplasty is feasible, and approximately how much I would need to prepare for out-of-pocket expenses? Thank you.

Benbengo, 40~49 year old female. Ask Date: 2020/04/22

Dr. Lai Zhaokang reply Neurosurgery

Hello: 1.
If the diagnosis of OPLL is confirmed and aligns with your symptoms, when planning for surgery, the general recommendation is posterior decompression surgery (laminoplasty with mini-plate or laminectomy with posterior minimally invasive screw fixation): mini-plates are usually out-of-pocket expenses, while minimally invasive screws can be applied for insurance coverage (but approval is not guaranteed).
2.
Generally, anterior surgery is not recommended for OPLL.
3.
During posterior surgery, minimally invasive techniques have limitations due to the long segment involved (C3-4-5-6-7) and the constraints of human anatomical dimensions.
The incision for posterior cervical surgery will inevitably be larger to thoroughly address the lesion.
4.
Costs: These will depend on the properties and quantity of the bone materials used, and may also involve the scope of insurance coverage.
Practically, you should discuss this with your surgeon.
Best wishes to you.

Understanding the treatment options for Ossification of the Posterior Longitudinal Ligament (OPLL) and their associated costs in the U.S. is crucial for making informed decisions about your healthcare. OPLL is a condition characterized by the calcification or ossification of the posterior longitudinal ligament in the cervical spine, which can lead to spinal cord compression and neurological symptoms.

Treatment Options
1. Posterior Decompression Surgery (Laminoplasty): This procedure involves the use of mini-plates to perform a laminoplasty, which helps to relieve pressure on the spinal cord. It is often recommended for patients with OPLL, especially when the ossification is extensive and causing significant symptoms. The surgery is performed from the back of the neck and aims to create more space for the spinal cord.

2. Anterior Cervical Discectomy and Fusion (ACDF): In cases where the OPLL is causing severe symptoms or if there is significant disc degeneration, an anterior approach may be taken. This involves removing the affected disc and replacing it with an artificial disc or a bone graft to stabilize the spine.

3. Artificial Disc Replacement: This is a more recent technique that allows for the preservation of motion in the cervical spine. It is typically considered for patients who have specific indications and may not be suitable for all OPLL cases.

4. Minimally Invasive Surgery: This approach can be used for both posterior and anterior surgeries, involving smaller incisions and potentially shorter recovery times. However, the feasibility of minimally invasive techniques depends on the specific anatomy and extent of the OPLL.

Estimated Costs
The costs associated with these procedures can vary widely based on several factors, including the complexity of the surgery, the hospital or surgical center, the surgeon's experience, and whether the procedure is performed on an inpatient or outpatient basis. Here’s a rough estimate of what you might expect:
- Posterior Decompression Surgery: The total cost can range from $30,000 to $80,000. This includes hospital fees, surgeon fees, anesthesia, and any necessary imaging studies.

- Anterior Cervical Discectomy and Fusion: This procedure can range from $20,000 to $60,000, depending on whether it involves a single or multiple levels of the cervical spine.

- Artificial Disc Replacement: This can be more expensive, typically ranging from $40,000 to $100,000, due to the cost of the artificial disc itself and the complexity of the surgery.

- Minimally Invasive Techniques: While these may reduce recovery time, they can sometimes be more expensive due to the specialized equipment and techniques used, with costs potentially exceeding $80,000.

Insurance and Out-of-Pocket Costs
If you have health insurance, it is essential to check with your provider regarding coverage for these procedures. Many insurance plans cover surgical interventions for OPLL, but the extent of coverage can vary. You may be responsible for copays, deductibles, and any non-covered services. If you are considering self-paying, it is advisable to discuss payment plans or financial assistance options with the hospital or surgical center.

Conclusion
In summary, the treatment options for OPLL include posterior decompression surgery, anterior cervical discectomy and fusion, and artificial disc replacement, with varying costs associated with each. Minimally invasive techniques may also be an option, depending on your specific case. It is crucial to have a detailed discussion with your healthcare provider about the best surgical approach for your condition, the expected outcomes, and the financial implications. This will help you make an informed decision that aligns with your health needs and financial situation.
