Abdominal issues?
Hello Doctor, since the end of last year, I have been experiencing a feeling of bloating and pressure in my upper left abdomen.
The discomfort is more of a dull ache rather than severe pain, but it does hurt when I press on the painful area.
Yesterday, I had pain near my left rib, but it is not painful today.
Occasionally, I experience brief anal pain, with one instance where the pain occurred multiple times in a day and lasted longer than usual, and it was more intense than normal.
Recently, I have not had much pain.
I usually drink less water, sit for long periods, do not exercise often, experience constipation, stay up late, and have bloating with gas.
After being constipated for a while, I sometimes have diarrhea, and recently I have had more soft stools, although the quantity is not much.
My family has a history of ovarian cancer and endometrial cancer, and I am concerned that this may be a tumor issue.
I hope the doctor can provide some answers.
Thank you!

More Info
Upper left abdominal pain can be a complex issue with various potential causes, and your symptoms warrant careful consideration. Based on your description, it seems you are experiencing a combination of discomfort, including a feeling of pressure and occasional sharp pain in the left upper abdomen, as well as some gastrointestinal symptoms such as constipation and changes in bowel habits. Additionally, your family history of ovarian and endometrial cancer understandably raises concerns about possible underlying malignancies.
Possible Causes of Upper Left Abdominal Pain
1. Gastrointestinal Issues:
- Gastritis or Peptic Ulcer Disease: Inflammation of the stomach lining or the presence of ulcers can cause dull, aching pain in the upper abdomen, particularly after meals. Symptoms may include bloating, nausea, and changes in appetite.
- Irritable Bowel Syndrome (IBS): Given your symptoms of constipation followed by diarrhea, IBS could be a possibility. This functional gastrointestinal disorder often presents with abdominal pain, bloating, and altered bowel habits.
- Diverticulitis: Although more common in the lower abdomen, diverticulitis can sometimes present with pain in the upper abdomen, especially if diverticula are present in the left colon.
2. Musculoskeletal Issues:
- Costochondritis: Inflammation of the cartilage connecting the ribs to the sternum can cause localized pain that may mimic abdominal pain. This is often exacerbated by certain movements or pressure.
- Muscle Strain: Prolonged sitting or poor posture can lead to muscle strain in the abdominal or back muscles, resulting in discomfort that may be perceived as abdominal pain.
3. Organ-Related Issues:
- Splenic Conditions: Problems with the spleen, such as splenomegaly (enlargement of the spleen) or splenic infarction, can cause pain in the left upper quadrant.
- Pancreatitis: Although typically associated with pain in the upper middle abdomen, inflammation of the pancreas can sometimes cause pain that radiates to the left side.
4. Reproductive System Concerns:
- Given your family history of gynecological cancers, it is essential to consider potential issues related to the ovaries or uterus. Conditions such as ovarian cysts or endometriosis can cause referred pain to the abdominal area.
Recommendations for Further Evaluation
1. Medical Evaluation: It is crucial to consult with a healthcare provider for a thorough evaluation. This should include a detailed history and physical examination, focusing on the abdominal area and any associated symptoms.
2. Imaging Studies: Depending on the initial assessment, imaging studies such as an abdominal ultrasound or CT scan may be warranted to visualize the organs in the upper abdomen and rule out any significant pathology.
3. Laboratory Tests: Blood tests, including a complete blood count (CBC) and liver function tests, can help identify any signs of infection, inflammation, or organ dysfunction.
4. Gastroenterology Referral: If gastrointestinal issues are suspected, a referral to a gastroenterologist may be beneficial for further evaluation, which could include endoscopy or colonoscopy if indicated.
5. Lifestyle Modifications: Improving hydration, increasing physical activity, and addressing dietary habits may help alleviate some gastrointestinal symptoms. Regular exercise and a balanced diet can promote better digestive health and reduce constipation.
Conclusion
While your symptoms are concerning, especially given your family history, many potential causes of upper left abdominal pain are benign and manageable. It is essential to seek medical attention to obtain a proper diagnosis and treatment plan tailored to your specific situation. Early intervention can lead to better outcomes, especially if any underlying conditions need to be addressed.
