One month after a Da Vinci laparoscopic myomectomy, the abdomen is still swollen?
Hello, Doctor.
I would like to ask you a question.
It has been one month since I underwent a robotic-assisted laparoscopic myomectomy (removal of one fibroid measuring 6.7 x 5.8 cm and three fibroids measuring 2 x 1 cm each; a total of 140 grams).
However, my abdomen still feels larger, firmer, and more distended than before the surgery (it feels very much like being pregnant), and I have not experienced any reduction in size.
Is this normal? P.S.
My bowel movements are normal, occurring daily, and there is no pain in my abdomen or at the incision site.
Dan Dan, 30~39 year old female. Ask Date: 2020/05/07
Dr. Wu Xianghui reply Obstetrics and Gynecology
It is recommended to return to the operating physician's clinic for an examination.

More Info
After undergoing a Da Vinci myomectomy, it is not uncommon for patients to experience abdominal swelling or a feeling of fullness in the abdomen for several weeks post-surgery. This can be attributed to several factors, including the surgical procedure itself, the body's healing response, and the accumulation of fluids in the abdominal cavity.
1. Postoperative Swelling: After any abdominal surgery, including a myomectomy, it is typical for the body to react with inflammation. This inflammation can lead to swelling, which may cause the abdomen to feel firm or distended. The surgical manipulation of tissues can also contribute to this sensation. In your case, since it has been just one month since the surgery, it is reasonable to expect some degree of swelling as your body continues to heal.
2. Fluid Accumulation: Another potential cause of abdominal swelling is the accumulation of serous fluid, which can occur after surgery. This is often referred to as a seroma. While seromas can be concerning, they are generally benign and may resolve on their own over time. If the swelling persists or worsens, it would be advisable to consult your surgeon for further evaluation.
3. Bowel Function: You mentioned that your bowel movements are normal, which is a positive sign. Post-surgical changes in bowel function can sometimes lead to bloating or discomfort, but the fact that you are having regular bowel movements suggests that your gastrointestinal system is functioning well.
4. Healing Process: The healing process can vary significantly from person to person. Factors such as your overall health, the extent of the surgery, and your body's individual healing response all play a role in how quickly swelling resolves. It is not unusual for some patients to experience prolonged swelling, especially if multiple fibroids were removed, as in your case.
5. When to Seek Help: While some swelling is normal, you should keep an eye on any changes. If you experience increased pain, fever, or if the swelling does not begin to decrease after a few weeks, it would be prudent to reach out to your healthcare provider. They may want to perform an examination or imaging studies to rule out any complications, such as infection or significant fluid accumulation.
6. Postoperative Care: In the meantime, ensure you are following your surgeon's postoperative care instructions. Staying hydrated, engaging in light activity as tolerated, and avoiding heavy lifting can all support your recovery. Gentle abdominal massages may also help alleviate some of the discomfort associated with swelling.
In conclusion, while some abdominal swelling one month after a Da Vinci myomectomy can be normal, it is essential to monitor your symptoms and maintain open communication with your healthcare provider. They can provide personalized advice and reassurance based on your specific situation. Remember, every patient's recovery journey is unique, and it is crucial to listen to your body as it heals.
