Cauliflower-like papules
Hello Doctor, I would like your help in examining my symptoms to determine whether it is pearly penile papules or genital warts.
I have a history of phimosis and had sexual intercourse about six months ago.
Recently, in the past two weeks, I started developing small bumps; previously, I had small ones that I washed off while showering, which left a wound.
Three days ago, I noticed a new growth.
Please see the photo linked below.
Thank you, Doctor.

Hello: This facility is not able to provide a diagnosis, but if it were me, I would go to the urology department for an examination.
The bumps appear to be more like small ones.
Wishing you good health.

Pearly penile papules (PPP) and genital warts are two distinct conditions that can appear on the penis, and understanding the differences between them is crucial for appropriate diagnosis and management.
Pearly Penile Papules (PPP) are small, dome-shaped, skin-colored or white bumps that typically appear around the corona of the glans (the head of the penis). They are a normal anatomical variant and are not associated with any disease or infection. PPP is common in men, particularly those who are uncircumcised, and they usually appear in rows. They are harmless and do not require treatment unless the individual desires removal for cosmetic reasons.
On the other hand, genital warts, caused by certain strains of the human papillomavirus (HPV), can appear as small, flesh-colored or gray growths that may be raised or flat. They can occur in clusters and may have a cauliflower-like appearance. Genital warts are sexually transmitted and can be associated with other health risks, including the potential for certain strains of HPV to lead to cancer. Unlike PPP, genital warts can cause discomfort, itching, or bleeding, especially if they are irritated.
In your case, you mentioned having a history of phimosis (tight foreskin) and recent sexual activity, which could increase the risk of developing genital warts. The fact that you have noticed new growths in the last two weeks is also significant. If the bumps are painful, itchy, or have changed in appearance, this could indicate a more serious condition, such as genital warts.
To differentiate between PPP and genital warts, consider the following:
1. Appearance: PPP are usually uniform in size and shape, while genital warts can vary widely in appearance and may be irregular.
2. Location: PPP are typically found around the glans, while genital warts can appear anywhere in the genital area, including the shaft of the penis, scrotum, and perianal region.
3. Symptoms: PPP are asymptomatic, whereas genital warts may cause itching, discomfort, or bleeding.
Given your symptoms and concerns, it is essential to consult a healthcare provider, preferably a urologist or dermatologist, for a proper examination and diagnosis. They may perform a physical examination and, if necessary, a biopsy to confirm the nature of the growths.
If it is determined that you have genital warts, treatment options include topical medications, cryotherapy (freezing the warts), laser therapy, or surgical removal. It's also important to discuss HPV vaccination, which can help prevent future infections with high-risk HPV strains.
In summary, while PPP is a benign condition, genital warts require medical attention due to their infectious nature and potential complications. Seeking professional advice will provide clarity and ensure that you receive the appropriate care.
