Throat clearing issues?

I have been experiencing a sensation of mucus stuck below my throat for quite a long time, which prompts me to cough using a "hmm-hmm" method.
It feels like there is phlegm, not a dry cough.
Sometimes, I do manage to bring up phlegm that is white.
I have consulted both Western and Traditional Chinese medicine.
The Western doctors said it is due to post-nasal drip, while the Chinese medicine practitioners suggested allergic cough and gastroesophageal reflux.
However, none of the treatments have been effective.
My company conducts annual health check-ups, and I have had chest X-rays that showed no abnormalities.
Occasionally, when I swallow saliva, I feel mucus deep in my throat.
The interesting thing is that I do not cough at all when I sleep.
I am unsure what the cause of this is.

Dr. Zheng Jueyi reply Otolaryngology

Hello: Chronic pharyngitis is commonly associated with post-nasal drip, smoking, or gastroesophageal reflux disease (GERD).
In addition to pharmacological treatment, dietary modifications to reduce gastroesophageal reflux, as well as sipping water instead of clearing the throat, are necessary for gradual improvement of the sensation of foreign body in the throat.
Wishing you safety and health.
Sincerely, Dr.

Understanding persistent throat mucus can be quite complex, as it often involves multiple underlying factors. Based on your description, it seems you are experiencing a sensation of mucus accumulation in your throat, which can be uncomfortable and lead to frequent coughing. This condition can be attributed to several potential causes, including post-nasal drip, allergies, gastroesophageal reflux disease (GERD), and even environmental factors.

Post-Nasal Drip: This is one of the most common causes of the sensation of mucus in the throat. It occurs when excess mucus accumulates in the nasal passages and drips down the back of the throat. This can be triggered by allergies, sinus infections, or colds. When you cough to clear your throat, it may be a reflex action to remove the mucus that is irritating your throat.

Allergies: Allergic reactions can lead to increased mucus production. Common allergens include pollen, dust mites, pet dander, and mold. If you have a history of allergies, it might be worthwhile to consider allergy testing and management strategies, such as antihistamines or avoiding known triggers.

Gastroesophageal Reflux Disease (GERD): GERD can cause stomach acid to flow back into the esophagus, leading to irritation and increased mucus production as a protective response. Symptoms of GERD can include heartburn, regurgitation, and a sensation of a lump in the throat. If you suspect GERD, lifestyle modifications such as avoiding certain foods, eating smaller meals, and not lying down immediately after eating can be beneficial. In some cases, medications like proton pump inhibitors may be prescribed.

Environmental Factors: Dry air, pollution, or exposure to irritants such as smoke can also contribute to throat discomfort and mucus production. Using a humidifier in your home, especially during dry seasons, can help alleviate these symptoms.

Other Considerations: It’s important to note that certain medical conditions can also lead to increased mucus production. Chronic respiratory conditions like asthma or chronic obstructive pulmonary disease (COPD) may present with similar symptoms. If you have been evaluated by both Western and Traditional Chinese Medicine practitioners without satisfactory results, it may be beneficial to seek a second opinion or consult an ear, nose, and throat (ENT) specialist for a more thorough examination.

Management Strategies: Here are some strategies that may help alleviate your symptoms:
1. Hydration: Staying well-hydrated can thin mucus, making it easier to clear.

2. Steam Inhalation: Inhaling steam from a hot shower or a bowl of hot water can help loosen mucus.

3. Saline Nasal Rinse: Using a saline solution to rinse your nasal passages can help clear out mucus and allergens.

4. Avoid Irritants: Stay away from smoke, strong odors, and other irritants that can exacerbate your symptoms.

5. Dietary Adjustments: If GERD is a concern, consider avoiding spicy, fatty, or acidic foods that can trigger reflux.

In conclusion, the sensation of mucus in the throat can stem from various causes, and addressing the underlying issue is key to finding relief. If your symptoms persist despite trying these strategies, it would be advisable to follow up with a healthcare provider for further evaluation and tailored treatment options.
