Consultation on Medications for Sleep and Panic Anxiety
Hello, Dr.
Lin.
I have been a patient with anxiety and panic for over 10 years.
I have been taking Rifuxin (Lifuyou) once a day, every night before bed.
However, recently I have been suffering from a stomach condition that has caused increased anxiety.
I have been having difficulty falling asleep (I usually only fall asleep around 5:00 AM and do not sleep well).
Today, I suddenly experienced unexplained dizziness, so I went to the emergency room.
After the examination, the doctor said everything seemed normal.
I mentioned to the doctor that my lack of sleep might be the cause.
The doctor prescribed me 3 tablets of Sominex (10 mg).
I would like to ask Dr.
Lin if it is safe to take this sleeping medication together with Rifuxin (Lifuyou), or if there is a specific order in which I should take them.
Since it was my first time taking a sleeping pill, my anxiety increased even more...
I sincerely request your help, Dr.
Lin.
Thank you.

Dr. Lin Mianzhang reply Psychiatry
Hello Mr.
Lin: Suemianos can be taken together with Rifampicin and Liyou, and the order of administration will not affect their efficacy.
A small number of people may experience sleepwalking or amnesia while taking this medication, but whether these side effects occur can only be determined after taking it.
If you are concerned, it is recommended to start with a lower dose, such as taking half a tablet to see how it goes.
If you have no issues and are able to sleep, then half a tablet is sufficient; if you still cannot sleep after taking it, you can increase the dose to one tablet.
However, if side effects are significant, it is best to refrain from taking it.
Wishing you a speedy recovery!

Dear Patient,
Thank you for reaching out with your concerns regarding your anxiety and sleep issues. It sounds like you have been dealing with these challenges for quite some time, and I commend you for seeking help. Based on your description, it appears that your anxiety has been exacerbated by your recent gastrointestinal issues, which can indeed contribute to sleep disturbances.
To address your primary question about the use of sleep aids, specifically Sominex (which I assume is the sleep medication you referred to), in conjunction with your current medication, Ritalin (which I believe is the medication you meant by "利福全 離憂"), it is essential to consider a few factors.
1. Medication Interactions: Generally, Sominex (diphenhydramine) is an antihistamine that can be used as a sleep aid. It is usually safe to take with many medications, including some antidepressants. However, since you are also taking Ritalin, which is a stimulant, it’s crucial to monitor how these medications interact in your body. While there are no direct contraindications, combining a stimulant with a sedative can sometimes lead to unpredictable effects. It is advisable to consult your prescribing physician or pharmacist to ensure that there are no specific concerns regarding your unique health profile.
2. Timing and Dosage: When taking multiple medications, the timing can be essential. Typically, it is recommended to take sleep aids shortly before bedtime to allow them to take effect. If you are taking Ritalin in the morning, it should not interfere with the sedative effects of Sominex taken at night. However, if you are feeling increased anxiety after taking the sleep medication, it may be beneficial to discuss this with your doctor. They might suggest adjusting the dosage or trying a different sleep aid that may be less likely to cause anxiety.
3. Underlying Anxiety: It’s important to recognize that anxiety itself can significantly impact sleep quality. If your anxiety is heightened, it can lead to a cycle of insomnia and increased anxiety, making it even more challenging to achieve restful sleep. Cognitive-behavioral therapy (CBT) for insomnia, mindfulness practices, and relaxation techniques can be beneficial in managing anxiety and improving sleep quality. These approaches can be used alongside medication for a more comprehensive treatment plan.
4. Consulting Your Doctor: Given your recent experience with dizziness and the ongoing challenges with sleep, I strongly encourage you to follow up with your healthcare provider. They can help assess whether your current treatment plan is effective or if adjustments are needed. It’s also essential to communicate any side effects or new symptoms you experience, as these can provide valuable insights into your treatment.
5. Lifestyle Modifications: In addition to medication, consider implementing lifestyle changes that promote better sleep hygiene. This includes establishing a regular sleep schedule, creating a calming bedtime routine, limiting screen time before bed, and avoiding caffeine and heavy meals in the evening. Regular physical activity can also help reduce anxiety and improve sleep quality.
In conclusion, while it may be safe to take Sominex with Ritalin, it is crucial to consult with your healthcare provider to ensure that this combination is appropriate for you. Addressing the underlying anxiety through therapy and lifestyle changes can also play a significant role in improving your sleep quality. Remember, you are not alone in this journey, and seeking help is a positive step towards better health.
Wishing you all the best on your path to recovery.
Sincerely,
