Foreskin issues
Hello Doctor: I have experienced inflammation of the glans and foreskin several times in the past few months, and it has mostly healed now.
However, I have the following questions:
1.
There is a spot under the foreskin that sometimes turns very red when I shower or retract the foreskin and then push it back.
Is this normal?
2.
After the second episode of inflammation, I feel that the foreskin has become tighter.
I can retract it while showering, but it is difficult to do so otherwise.
The situation fluctuates; sometimes I can't retract it at all during a shower, and it even feels like the foreskin is somewhat stuck to the glans.
What should I do if I have difficulty retracting the foreskin? The doctor mentioned it could be chronic inflammation and advised me to observe it for a while, but if it doesn't improve, surgery may be necessary.
I am quite fearful of surgery; aside from improving hygiene, what else can I do?
3.
After several episodes of inflammation, I have noticed that the tip of my foreskin sometimes appears wrinkled and the skin feels loose.
Is this condition normal?
I appreciate your time in addressing these questions.
Thank you!

Hello: Redness is often due to inflammation, and it can take several months to resolve, as the foreskin is prone to moisture, making inflammation more likely to occur.
2) After urination, it is important to dry the area thoroughly; it should not remain wet.
Clean with warm water once or twice a day, and avoid using any cleansing products.
If redness persists, antibiotics may be necessary (as determined by a physician).
3) It is normal as long as there is no redness.
After it resolves, pay extra attention to cleanliness and dryness to prevent recurrence.
Wishing you good health.

Understanding foreskin issues, particularly inflammation and tightness, is essential for maintaining genital health. Your concerns about recurrent inflammation, changes in the appearance of the foreskin, and tightness are valid and warrant careful consideration.
1. Redness Under the Foreskin: It is not uncommon for the area beneath the foreskin to appear red after washing or when the foreskin is retracted. This redness can be a result of irritation from friction, soap, or even the water itself. However, if the redness persists or is accompanied by pain, swelling, or discharge, it may indicate an underlying issue such as inflammation or infection. In such cases, it is advisable to consult a healthcare provider for a thorough evaluation.
2. Tightness of the Foreskin: The sensation of tightness in the foreskin, especially after episodes of inflammation, can be concerning. This condition, known as phimosis, can occur when the foreskin becomes less elastic due to scarring or chronic inflammation. If you find it difficult to retract the foreskin, especially when not in a warm, moist environment (like during a shower), it may be beneficial to gently stretch the foreskin over time. This can be done by applying a mild topical steroid cream (as prescribed by a doctor) to help reduce inflammation and improve elasticity. However, if the tightness persists or worsens, surgical options such as circumcision or preputioplasty may be necessary. It is understandable to feel apprehensive about surgery, but discussing your fears with a healthcare provider can help you weigh the risks and benefits.
3. Wrinkling of the Foreskin: The appearance of wrinkling or sagging skin at the tip of the foreskin can occur after repeated inflammation. This may be due to changes in skin elasticity or scarring. While some degree of wrinkling can be normal, especially after inflammation, significant changes in texture or appearance should be evaluated by a healthcare professional. They can determine if these changes are a result of chronic inflammation or if they indicate a more serious condition.
Care Tips:
- Hygiene: Maintaining proper hygiene is crucial. Gently clean the area with warm water and mild soap, ensuring that you rinse thoroughly to avoid irritation from soap residue.
- Moisturizing: Applying a gentle, fragrance-free moisturizer can help keep the skin supple and reduce irritation.
- Avoid Irritants: Be cautious with products that may irritate the area, such as scented soaps, lotions, or lubricants.
- Monitor Symptoms: Keep track of any changes in symptoms, including the frequency and severity of inflammation, and report these to your healthcare provider.
- Consultation: If symptoms persist or worsen, do not hesitate to seek medical advice. A urologist or dermatologist specializing in genital health can provide tailored recommendations and treatment options.
In conclusion, while some changes in the foreskin may be normal, persistent symptoms should not be ignored. Open communication with your healthcare provider about your concerns and treatment options is essential for achieving the best outcomes for your genital health.
