Heart Palpitations: Concerns for Patients with Hypothyroidism - Cardiology

Could you please provide more details about the heart problem?

Hello, Director Huang: I have hypothyroidism and also experience palpitations.
Recently, I have been taking a heart rate stabilizing medication, Nankuang Nengdeshuang sustained-release capsules, 120 mg, once daily.
In the mornings, after the medication's effect wears off, my heart rate can rise to around 104–112 beats per minute.
I have undergone a chest X-ray and a 24-hour ECG, and both the initial ECG during palpitations and the follow-up ECG were normal.
After taking the medication, my heart rate slows down, and after 6 PM, it typically drops to around 80–90 beats per minute.
When I check my pulse, it becomes slower, and I am concerned about the possibility of my heart stopping.
I kindly request your detailed response.
Thank you.
🙏🙏

Dr. Huang Yinghe reply Cardiology

Hello: Nengde Shuang is a calcium channel blocker medication, primarily used to lower heart rate and reduce symptoms of chest tightness and palpitations.
At normal dosages, it does not cause cardiac arrest; maintaining a heart rate above 60 beats per minute while on the medication is not a problem.
If you experience any discomfort while taking the medication, please discuss it with your physician.

Heart palpitations can be a concerning symptom for many patients, especially those with underlying conditions such as hypothyroidism. In your case, it is important to understand the relationship between your thyroid function, heart rate, and the medications you are taking.

Hypothyroidism, or an underactive thyroid, can lead to a variety of symptoms, including fatigue, weight gain, and depression. It can also affect the cardiovascular system. Patients with hypothyroidism may experience bradycardia (a slower than normal heart rate) and other heart rhythm abnormalities. However, it is also possible for hypothyroid patients to experience palpitations, particularly if they are on medication that affects heart rate or if their thyroid hormone levels fluctuate.

In your situation, you mentioned that you are taking a medication called "南光 能得爽緩釋膠囊" (Nangguang Nengdeshuang Sustained Release Capsule) at a dosage of 120 mg once daily. This medication is likely intended to help stabilize your heart rate. It is not uncommon for patients to experience variations in heart rate throughout the day, especially in relation to medication timing and effectiveness. The fact that your heart rate increases to 104-112 beats per minute in the morning and then stabilizes to 80-90 beats per minute in the evening suggests that your medication may be wearing off by the morning, leading to a temporary increase in heart rate.

Regarding your concern about the possibility of your heart stopping, it is important to note that while palpitations can be alarming, they are often benign, especially if your heart has been evaluated through tests such as chest X-rays and 24-hour Holter monitoring, which you indicated were normal. A normal ECG and Holter monitor suggest that there are no significant arrhythmias or structural heart issues at this time. However, if you experience severe palpitations, dizziness, or other concerning symptoms, it is crucial to seek medical attention promptly.

It is also worth considering that anxiety and stress can exacerbate feelings of palpitations. If you find that your heart rate increases in response to stress or anxiety, it may be beneficial to explore relaxation techniques or speak with a mental health professional about managing anxiety.

In summary, while your heart rate fluctuations and palpitations can be concerning, the normal results from your heart evaluations are reassuring. It is essential to continue monitoring your symptoms and maintain open communication with your healthcare provider. If you have ongoing concerns about your heart rate or the effects of your medication, do not hesitate to discuss these with your doctor, who can provide personalized guidance based on your specific health needs.
