Post-coital penile pain?
Hello, doctor.
Recently, after sexual intercourse, my boyfriend experiences pain in his penis when he gets an erection (either on the same day or one or two days later).
I would like to inquire about the possible reasons for this (he claims that resting for a while helps).
Just to note, there are no issues with dryness, no excessive frequency (for example, not more than twice a day), he is not circumcised but the glans is fully exposed during an erection, there are no unusual positions, and no DIY activities.

Prolonged erections can occasionally be accompanied by pain.
As long as there is no trauma and it is not an unusually prolonged erection, if the symptoms alleviate, there is no need for concern.
However, if there are any doubts, it is still advisable to consult a clinic.

Post-sexual activity pain in the penis can be a concerning issue for many men, and understanding the potential causes is essential for addressing any discomfort. Based on the information provided, there are several factors that could contribute to your boyfriend's experience of pain after sexual activity.
1. Prolonged Erection: One of the most common reasons for post-sexual activity pain is a prolonged erection, known medically as priapism. While priapism typically refers to an erection lasting more than four hours, even shorter durations can lead to discomfort if the erection is sustained without ejaculation. The pressure from blood engorgement can cause pain in the penis and surrounding areas, particularly if the erection is maintained for an extended period without release.
2. Muscle Tension: Sexual activity often involves significant muscle engagement, including the pelvic floor muscles. If these muscles are tense or fatigued, they can lead to discomfort or pain in the genital area post-activity. This is especially true if the sexual activity was vigorous or if there was a lot of thrusting involved.
3. Nerve Sensitivity: The genital area is rich in nerve endings, and sometimes, after sexual activity, the nerves can become overly sensitive. This heightened sensitivity can lead to pain or discomfort during subsequent erections. If your boyfriend is experiencing this pain consistently after sexual activity, it may be worth exploring whether there is an underlying nerve issue.
4. Infection or Inflammation: Although your boyfriend has not reported any signs of infection, such as unusual discharge or fever, it is still a possibility that should not be overlooked. Conditions such as prostatitis (inflammation of the prostate) or epididymitis (inflammation of the epididymis) can cause pain in the genital area and may not always present with obvious symptoms. A visit to a healthcare provider for a thorough examination could help rule out these conditions.
5. Psychological Factors: Anxiety or stress related to sexual performance can also manifest as physical symptoms, including pain. If your boyfriend is feeling anxious about sexual activity, this could contribute to muscle tension and discomfort.
6. Lack of Adequate Arousal or Lubrication: While you mentioned that dryness is not an issue, it's important to ensure that both partners are adequately aroused and that sufficient lubrication is used during sexual activity. Insufficient lubrication can lead to friction, which may cause pain during or after sex.
7. Post-Ejaculatory Pain: Some men experience pain after ejaculation, which can be due to muscle contractions or other physiological responses. This is often temporary and resolves on its own but can be uncomfortable.
Given that your boyfriend reports that the pain resolves after resting, it may not be a cause for immediate concern. However, if the pain persists, worsens, or is accompanied by other symptoms, it would be advisable for him to consult a healthcare professional. A urologist can perform a thorough evaluation, which may include a physical examination, urine tests, or imaging studies, to determine the underlying cause of the pain.
In summary, while post-sexual activity pain can be attributed to several benign factors, it is essential to monitor the situation. Encouraging your boyfriend to communicate openly about his discomfort and to seek medical advice if necessary can help ensure that any potential issues are addressed promptly.
