Why does the penis experience cramping pain after intercourse?
Recently, after each sexual intercourse with my wife, I experience a throbbing pain in my penis that lasts for about 1-2 days.
What could be the cause of this? Thank you!

Dr. Chen Zhirong reply Urology
Mr.
Wu: For a man, achieving and maintaining an erection relies on the full expansion of the corpora cavernosa and the continuous influx of blood.
After ejaculation, the blood is expelled through the penile veins in conjunction with the relaxation of the corpora cavernosa.
Therefore, any structural issues with the corpora cavernosa (such as inflammation or even fibrosis) or obstruction of the vascular outflow may lead to the symptoms you described.
I recommend that you consult a specialist for a thorough evaluation to prevent further deterioration of your condition.

Experiencing pain in the penis after intercourse can be concerning and may stem from various causes. It’s essential to understand that while occasional discomfort can be normal, persistent or severe pain should be evaluated by a healthcare professional. Here are some common reasons why you might be experiencing this pain:
1. Trauma or Injury: During intercourse, the penis can undergo physical stress or trauma, especially if there is vigorous movement or if positions that exert pressure are used. This can lead to micro-tears in the penile tissue, resulting in pain that may last for a day or two.
2. Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, which can cause curved, painful erections. If you notice a change in the shape of your penis or if the pain is accompanied by a noticeable curve, it’s advisable to consult a doctor.
3. Infections: Sexually transmitted infections (STIs) such as gonorrhea, chlamydia, or herpes can cause pain during or after intercourse. Other infections, such as urinary tract infections (UTIs) or prostatitis (inflammation of the prostate), can also lead to discomfort. Symptoms may include burning during urination, unusual discharge, or fever.
4. Allergic Reactions: Sometimes, the materials in condoms, lubricants, or even soaps can cause allergic reactions or irritation, leading to pain after intercourse. If you’ve recently changed any products, consider reverting to previous ones to see if the pain subsides.
5. Erectile Dysfunction or Premature Ejaculation: Psychological factors related to performance anxiety can lead to tension and discomfort during intercourse. If you are experiencing anxiety about sexual performance, it may manifest as physical pain.
6. Overexertion: Engaging in sexual activity for an extended period or with excessive intensity can lead to soreness. Just like any physical activity, the muscles involved can become fatigued, leading to discomfort afterward.
7. Skin Conditions: Conditions such as balanitis (inflammation of the glans) or dermatitis can cause pain and discomfort in the penis. If you notice redness, swelling, or unusual discharge, it’s important to seek medical advice.
8. Underlying Medical Conditions: Certain medical conditions, such as diabetes or cardiovascular issues, can affect sexual health and may lead to pain during or after intercourse. If you have other symptoms or a history of such conditions, it’s crucial to discuss these with your healthcare provider.
Recommendations:
- Consult a Healthcare Professional: If the pain persists or worsens, it’s essential to see a urologist or your primary care physician. They can perform a physical examination and may recommend tests to determine the underlying cause.
- Keep a Symptom Diary: Note when the pain occurs, its intensity, and any other symptoms you may experience. This information can help your doctor diagnose the issue more effectively.
- Practice Safe Sex: Using condoms can help prevent STIs and reduce irritation from allergens.
- Consider Lubrication: If dryness is an issue, using a water-based lubricant can help reduce friction during intercourse, potentially alleviating pain.
In summary, while occasional discomfort after intercourse can be normal, persistent pain warrants medical evaluation to rule out serious conditions. Taking proactive steps to address any underlying issues can help ensure a healthy and enjoyable sexual experience.
