Do I still need to exfoliate while using tretinoin?
Hello, doctor.
I would like to ask a few questions.
I am a male with combination skin that tends to be oily and prone to acne.
I have been using Tretinoin (Tifuli) daily as part of my skincare routine, and it has significantly improved my acne over time.
My first question is whether I still need to use exfoliating products occasionally, and if that would conflict with my current regimen.
My second question is about my cleansing routine.
In the morning, I wash my face with a cleanser, and in the afternoon, after playing sports and sweating, I only rinse my face with water.
Then, in the evening, I use the cleanser again.
Is this the correct approach, or should I wash my face with the cleanser after exercising and just use water in the evening? I am concerned that using the cleanser too frequently might dry out my skin and be detrimental to my face.
Thank you for your guidance.

No need for additional exfoliation.
You can continue to wash your face as you normally do.

More Info
When using tretinoin (commonly known as retinoic acid) for acne treatment, it's important to understand how it interacts with your skin and whether additional skincare practices, such as exfoliation, are beneficial or potentially harmful.
Exfoliation While Using Tretinoin
1. Do You Need Exfoliation?
Tretinoin is a powerful topical retinoid that promotes cell turnover, which means it helps your skin shed dead skin cells and encourages the growth of new, healthy skin. Because of this, many people find that they do not need to exfoliate as frequently while using tretinoin. However, if you have particularly oily skin or if you notice that your skin feels rough or has a buildup of dead skin cells, gentle exfoliation can be beneficial.
It is crucial to choose the right type of exfoliant. Chemical exfoliants, such as those containing alpha hydroxy acids (AHAs) or beta hydroxy acids (BHAs), can be a good option as they tend to be less abrasive than physical scrubs. However, you should avoid using these exfoliants on the same days you apply tretinoin, as this can lead to increased irritation, redness, and peeling. Instead, consider using exfoliants on alternate days or once a week, depending on your skin's tolerance.
2. Potential Conflicts:
The combination of tretinoin and exfoliating products can sometimes lead to over-exfoliation, which can compromise your skin barrier and lead to irritation. If you decide to incorporate exfoliation into your routine, start slowly and monitor your skin's response. If you experience excessive dryness or irritation, it may be best to reduce the frequency of exfoliation or discontinue it altogether.
Cleansing Routine
1. Your Current Routine:
You mentioned that you wash your face with a cleanser in the morning, use only water after exercising in the afternoon, and then wash your face again with a cleanser at night. This routine is generally acceptable, especially if you are concerned about over-drying your skin.
2. Should You Change Your Routine?
After exercising, if you are sweating and your skin feels oily or dirty, it is advisable to use a gentle cleanser rather than just water. This will help remove sweat, oil, and any impurities that can contribute to breakouts. However, if you find that using a cleanser twice a day is drying out your skin, you might consider using a gentle, hydrating cleanser that won’t strip your skin of its natural oils.
If you prefer to stick with just water after exercising, ensure that you are using a good moisturizer afterward to maintain hydration. This can help prevent your skin from becoming too dry, especially when using tretinoin, which can also be drying.
Conclusion
In summary, while using tretinoin, you may not need to exfoliate as often due to its natural exfoliating properties. If you choose to exfoliate, opt for gentle chemical exfoliants and avoid using them on the same days as tretinoin. Regarding your cleansing routine, consider using a gentle cleanser after exercising to remove sweat and oil, while being mindful of your skin's hydration levels. Always listen to your skin and adjust your routine based on how it reacts to these products. If you have any concerns or experience persistent irritation, consulting with a dermatologist is advisable for personalized recommendations.
