While doing DIY, there is a sudden brief sharp pain (lasting less than a second)?
Hello Doctor, recently I experienced a sudden brief sharp pain (lasting less than a second, felt near the glans) while masturbating just before ejaculation.
After that, I continued to masturbate until ejaculation without experiencing that sensation again.
Interestingly, I do not feel this sharp pain at all during intercourse with my girlfriend; it only occurs during masturbation.
At other times, I have no discomfort (urine flow is normal, and there is no increased frequency of urination).
I would like to ask what might be causing these symptoms.
Note: I am currently taking antihypertensive medication for glomerulonephritis.
Thank you.

The possible causes of the symptoms you described include trauma, infection, stones, and even referred pain from stones located at the ureterovesical junction.
It is recommended that you undergo further examination.

More Info
Experiencing brief sharp pain during masturbation, particularly localized around the glans (the head of the penis), can be concerning and may have various underlying causes. Given that this pain occurs only during masturbation and not during sexual intercourse with your partner, it suggests that the issue may be related to the mechanics or technique of masturbation rather than a systemic or serious medical condition.
Possible Causes of Sharp Pain During Masturbation
1. Friction or Irritation: One of the most common causes of sharp pain during masturbation is friction. If the technique involves excessive pressure or speed, it can lead to irritation of the sensitive skin on the glans. This is especially true if lubrication is insufficient. Consider using a water-based lubricant to reduce friction and see if that alleviates the pain.
2. Nerve Sensitivity: The glans is densely packed with nerve endings, making it highly sensitive. Some individuals may experience heightened sensitivity during masturbation, which can lead to sharp pain. This sensitivity can vary based on factors such as arousal levels, anxiety, or even the time of day.
3. Infection or Inflammation: Although you mentioned no other symptoms such as changes in urination or discharge, it’s still worth considering the possibility of a mild infection or inflammation, such as balanitis (inflammation of the glans) or urethritis (inflammation of the urethra). These conditions can cause localized pain and may not always present with obvious symptoms.
4. Psychological Factors: Anxiety or stress can sometimes manifest physically, leading to muscle tension or heightened sensitivity during sexual activities. If you are feeling anxious about the act of masturbation itself, this could contribute to the pain.
5. Underlying Medical Conditions: Given your history of hypertension and treatment for nephritis, it’s important to consider that certain medications can have side effects that affect sexual function or sensation. If you are on blood pressure medications, they may influence blood flow or nerve sensitivity, potentially contributing to the pain.
Recommendations
- Monitor Symptoms: Keep track of when the pain occurs, its intensity, and any other accompanying symptoms. This information can be helpful for your healthcare provider.
- Adjust Technique: Experiment with different techniques, speeds, and levels of pressure during masturbation. Incorporating lubrication can significantly reduce friction and discomfort.
- Consult a Healthcare Provider: If the pain persists or worsens, it would be wise to consult a urologist or your primary care physician. They can perform a thorough examination and may recommend tests to rule out infections or other underlying conditions.
- Discuss Medications: Since you are on blood pressure medication, discuss with your doctor whether these could be contributing to your symptoms. They may adjust your treatment plan if necessary.
- Psychological Support: If anxiety is a concern, consider speaking with a mental health professional. They can provide strategies to manage anxiety and improve your overall sexual health.
In conclusion, while brief sharp pain during masturbation can be alarming, it is often related to mechanical factors or sensitivity rather than a serious medical issue. However, given your medical history, it’s important to approach this symptom with caution and seek professional advice if necessary.
