A one-second pain sensation during masturbation?

Hello Doctor, recently (starting from mid-April), I have noticed a sudden sharp pain that lasts less than a second while masturbating (the pain occurs around the mid-shaft of the penis to the glans, which I have never experienced before).
It feels somewhat like a stone trying to pass but then retracting.
After that, I continue to masturbate, and this sensation does not occur again, meaning I only experience this sensation once at most during each session.
Additionally, I do not experience this sensation during intercourse (my masturbation technique is similar to friction).
Aside from the aforementioned symptoms, I have no other discomfort (such as frequent urination, and my urinary flow is normal).
However, I have been taking ARB antihypertensive medication since mid-December due to IgA nephropathy.
(In November of last year, a company health check showed my PSA level was 0.93 ng/ml [normal], but in April of this year, an ultrasound during a company health check indicated a suspected stone measuring 0.27 cm x 0.15 cm).
I would like to ask the doctor what these symptoms might indicate.
Thank you, Doctor.

Hello: 1) If there are stones, it is important to check if they have been expelled.
2) Pain during masturbation is usually not present at other times and is likely related to nerve reflexes.
3) Your condition is probably not significantly related to the stones; it may require lifestyle adjustments, exercise, moderate DIY activities, and increased water intake to see if there is improvement.
You might also consider trying B vitamins.
Wishing you good health.

More Info

The experience of brief pain during masturbation, particularly the sharp sensation you describe occurring in the mid-shaft of the penis towards the glans, can be concerning, especially when it is a new symptom. While I cannot provide a definitive diagnosis without a physical examination, I can outline some potential causes and considerations based on your description and medical history.

1. Urethral Sensitivity or Irritation: The urethra, which runs through the penis, can become sensitive or irritated due to various factors, including friction during masturbation, changes in lubrication, or even minor infections. If the sensation is fleeting and does not recur frequently, it may simply be a temporary irritation.

2. Urethral Stricture or Lesion: A stricture (narrowing) or lesion in the urethra can cause sharp pain during certain activities, including masturbation. This could be due to previous trauma, infections, or other underlying conditions. If the pain persists or worsens, it would be advisable to consult a urologist for further evaluation.

3. Kidney Stones: Given your history of IgA nephropathy and the recent ultrasound indicating a possible stone, it is worth considering that the pain could be related to kidney stones. While you mentioned that the pain occurs only during masturbation and not during sexual intercourse, kidney stones can sometimes cause referred pain that may be felt in the genital area. The sharp pain you experience could be a result of the stone moving or causing irritation in the urinary tract.

4. Psychological Factors: Sometimes, anxiety or stress related to sexual activity can manifest as physical symptoms. If you are concerned about your health or the implications of your kidney condition, this could contribute to heightened sensitivity or discomfort during sexual activities.

5. Infection: Although you do not report any urinary symptoms such as frequency or urgency, infections of the urinary tract or sexually transmitted infections can cause localized pain. If there are any changes in your urinary habits or if the pain persists, testing for infections may be warranted.

6. Prostate Issues: While your PSA levels are normal, issues related to the prostate, such as prostatitis, can sometimes cause discomfort during sexual activities. This is less likely given your symptom profile, but it is still a consideration.

Given your medical history, particularly with IgA nephropathy and the potential for kidney stones, it is essential to monitor your symptoms closely. If the pain persists, increases in frequency, or is accompanied by other symptoms (such as changes in urination, blood in urine, or persistent discomfort), I recommend seeking a consultation with a urologist. They may perform imaging studies or other diagnostic tests to rule out any underlying conditions.

In the meantime, consider adjusting your technique during masturbation to see if that alleviates the discomfort. Using adequate lubrication and being mindful of the pressure applied can help reduce irritation.
Overall, while occasional sharp pain during masturbation can occur for various benign reasons, persistent or recurrent symptoms warrant further investigation to ensure there are no underlying health concerns.
