Nasal congestion all day?

Hello Doctor: I would like to ask about a problem I've been experiencing since March of this year.
I have been experiencing nasal congestion on the right side of my nose (about one-third of my nasal passage is blocked all day).
While riding my bike, I feel like there are round obstructions in both nostrils.
When I take a shower, my right nostril becomes completely blocked, and it remains blocked while I sleep.
However, when I turn over, the congestion quickly shifts to the other side.
During this period, both nostrils have been swollen to the point where it's almost impossible to breathe.
I tried taking antihistamines, but the effect was only moderate.
Around mid-April to early May, the congestion worsened significantly; one side was almost completely blocked, while the other side could only allow about half of the airflow.
While riding my bike, my right nostril remained blocked, and the same situation occurred during showering and sleeping.
Now, it feels particularly severe, and I almost have to breathe through my mouth.
Additionally, the congestion seems to take longer to switch sides when I turn over, and even lying down causes significant blockage.
I would like to ask the doctor what could be causing this issue.
Thank you.

Hello: It is common for nasal septum deviation and hypertrophic rhinitis to cause enlargement of the inferior turbinates, leading to a narrowing of the nasal cavity.
If you are experiencing persistent nasal congestion, it is advisable to visit an otolaryngologist for a thorough examination.
Wishing you safety and good health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan General Hospital, Ministry of Health and Welfare.

More Info

Persistent nasal congestion can be a frustrating and debilitating condition, impacting your quality of life significantly. Based on your description, it seems that you are experiencing unilateral nasal congestion, which can be caused by a variety of factors. Understanding the underlying causes is crucial for effective treatment.

Possible Causes of Persistent Nasal Congestion
1. Allergic Rhinitis: This is one of the most common causes of nasal congestion. Allergens such as pollen, dust mites, pet dander, and mold can trigger an immune response, leading to inflammation and swelling of the nasal mucosa. Symptoms often include sneezing, itching, and a runny nose, but congestion can be predominant, especially if you are exposed to allergens frequently.

2. Non-Allergic Rhinitis: This condition can occur without an allergic trigger and may be due to irritants like smoke, strong odors, or changes in weather. Non-allergic rhinitis can also be influenced by hormonal changes, medications, or even stress.

3. Structural Abnormalities: Conditions such as a deviated septum or nasal polyps can lead to chronic nasal obstruction. A deviated septum can cause one side of the nose to be more congested than the other, while nasal polyps—soft, painless growths on the lining of your nasal passages—can obstruct airflow and lead to persistent congestion.

4. Sinusitis: Chronic sinusitis, which can be either acute or chronic, often presents with nasal congestion, facial pain, and pressure. It can result from infections, allergies, or structural issues in the nasal passages. If you have had recurrent sinus infections, this could be a contributing factor.

5. Vasomotor Rhinitis: This type of rhinitis is characterized by nasal congestion due to changes in temperature, humidity, or exposure to irritants. It does not involve an allergic response but can cause significant discomfort.

Recommended Solutions
1. Consult an ENT Specialist: Given the duration and severity of your symptoms, it is advisable to consult an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination, which may include nasal endoscopy or imaging studies (like a CT scan) to assess for structural abnormalities or sinus issues.

2. Allergy Testing: If allergic rhinitis is suspected, allergy testing can help identify specific triggers. Once identified, avoiding these allergens and possibly using antihistamines or nasal corticosteroids can provide relief.

3. Nasal Corticosteroids: These are effective in reducing inflammation in the nasal passages and can help alleviate congestion. They are often the first-line treatment for both allergic and non-allergic rhinitis.

4. Saline Nasal Irrigation: Regularly rinsing your nasal passages with saline solution can help clear mucus and allergens, providing symptomatic relief.

5. Address Structural Issues: If structural abnormalities like a deviated septum or nasal polyps are identified, surgical options may be considered. Procedures such as septoplasty or polypectomy can restore normal airflow and alleviate chronic congestion.

6. Lifestyle Modifications: Avoiding known irritants, using air purifiers, and maintaining good hydration can also help manage symptoms.

7. Medication Review: If you are taking any medications that may contribute to nasal congestion (such as certain blood pressure medications), discuss alternatives with your healthcare provider.

Conclusion
Persistent nasal congestion can stem from various causes, and identifying the underlying issue is key to effective treatment. A comprehensive evaluation by an ENT specialist will help determine the best course of action tailored to your specific situation. In the meantime, implementing some of the suggested strategies may provide temporary relief and improve your overall comfort.
