Postoperative issues following a tonsillectomy?
Hello Doctor: Last month, a family member underwent trabeculectomy on their right eye due to elevated intraocular pressure from iritis.
Recently, they reported seeing a line in the lower middle part of their vision, resembling a seam through glass, particularly noticeable when facing bright light.
This line appears to reflect in the lower middle area of their vision and moves with their eye, but it is not like floaters, as it does not drift.
What could this issue be? Additionally, their left eye was previously normal (with intraocular pressure consistently between 14-17, never exceeding 20, and no history of iritis).
After the trabeculectomy on the right eye, the eyelids of the left eye became swollen (allergic reaction), which subsided after about three days.
However, starting the day after the surgery, the intraocular pressure in the left eye significantly increased and has been around 20-22 for about a month.
What could be the reason for the left eye's pressure increasing after surgery on the right eye? Thank you!

Hello Mr.
HELP: During the trabeculectomy, it is necessary to create a drainage hole on the iris at the surgical site.
If the drainage hole is larger, it may cause light interference, which your family members likely observed.
Theoretically, after surgery on the right eye, the intraocular pressure in the left eye should not increase.
However, for the actual situation, it would be best to consult the attending physician.

More Info
After undergoing a trabeculectomy, it is not uncommon for patients to experience various visual disturbances and changes in intraocular pressure (IOP). Your family member's symptoms, including the perception of a line or seam in their vision, particularly when looking at bright lights, could be attributed to several factors related to the surgery and the healing process.
Visual Disturbances Post-Surgery
1. Light Reflection and Visual Artifacts: The sensation of seeing a line or seam, especially in bright light, may be due to changes in the cornea or the lens, or it could be a result of the surgical procedure itself. During trabeculectomy, the surgeon creates a new drainage pathway for aqueous humor to reduce IOP, which can sometimes lead to temporary optical disturbances. If the line is particularly noticeable when looking at bright lights, it might be related to how light is refracted through the cornea or lens, especially if there are any irregularities or swelling post-surgery.
2. Corneal Edema: Post-operative corneal edema (swelling) can also cause visual disturbances, including blurriness or the perception of lines. This condition can occur due to fluid accumulation in the cornea, which may be exacerbated by the surgical trauma or changes in IOP.
3. Intraocular Pressure Fluctuations: After trabeculectomy, it is crucial to monitor IOP closely, as fluctuations can lead to visual disturbances. If the IOP is too high or too low, it can affect the optic nerve and overall vision. The perception of visual artifacts may be a sign that the IOP is not stable.
Increased Eye Pressure in the Left Eye
Regarding the increased IOP in the left eye after the right eye surgery, there are a few potential explanations:
1. Sympathetic Reaction: Sometimes, surgical trauma to one eye can lead to a sympathetic reaction in the other eye, causing changes in IOP. This phenomenon is not fully understood but can occur after ocular surgery.
2. Inflammation: Post-operative inflammation can affect both eyes, especially if there is a systemic inflammatory response. This inflammation can lead to increased IOP in the unaffected eye.
3. Medication Effects: If your family member is using any topical medications post-surgery, such as steroids, these can also influence IOP. Steroids are known to potentially increase IOP, especially in susceptible individuals.
4. Underlying Conditions: If the left eye had any pre-existing conditions that were previously asymptomatic, the stress of surgery on the right eye could have exacerbated these issues, leading to increased IOP.
Recommendations
1. Follow-Up with an Eye Specialist: It is essential for your family member to have a follow-up appointment with their ophthalmologist. They may need a comprehensive eye exam, including IOP measurements and possibly imaging studies to assess the optic nerve and the overall health of both eyes.
2. Monitor Symptoms: Keep track of any changes in vision, pain, or discomfort. If symptoms worsen, it is crucial to seek medical attention promptly.
3. Medication Review: Discuss with the ophthalmologist any medications being used post-surgery to ensure they are appropriate and not contributing to increased IOP.
4. Avoid Eye Strain: Encourage your family member to avoid activities that may strain their eyes, such as prolonged screen time, until their symptoms are evaluated and managed.
In conclusion, while some visual disturbances and changes in IOP can be expected after trabeculectomy, persistent or worsening symptoms warrant further investigation. It is crucial to maintain open communication with the healthcare provider to ensure the best possible outcome.
