Postoperative symptoms of cataract surgery?

Hello, Doctor.
I had cataract surgery (monofocal) on my left eye over a week ago, and I have two questions I would like to ask, thank you: 1.
My eye shakes continuously when I focus on objects; what could be the reason for this? 2.
The day after the surgery, my intraocular pressure was measured at 14, but when measured every three days afterward, it increased to 25 and 26 and hasn't decreased.
The doctor is also puzzled; could this be a complication? Why is this happening? I do not have diabetes or glaucoma.

Karen, 40~49 year old female. Ask Date: 2015/07/26

Dr. Gao Pilin reply Ophthalmology

Hello Karen: 1.
The shaking may be due to the intraocular lens being unstable within the capsular bag, which should improve once it stabilizes.
2.
Elevated intraocular pressure may be a temporary condition post-surgery; it can be managed with ocular hypotensive medications, and close monitoring of changes is necessary to adjust the treatment plan accordingly.
Taiwan E-Hospital Puzi Hospital Ophthalmology: Monday to Friday from 8:00 AM to 12:00 PM and Wednesday afternoon from 1:00 PM to 4:30 PM.
We hope to have the opportunity to serve you.

Hello! Thank you for reaching out with your concerns regarding your post-cataract surgery symptoms. It's understandable to have questions after such a significant procedure, and I’m here to help clarify some of the issues you’re experiencing.

1. Eye Movement and Shaking: The shaking or trembling of your eye when focusing on objects can be attributed to several factors. One common reason is that the intraocular lens (IOL) may not be fully stabilized within the capsular bag, which is the thin membrane that holds the lens in place. If the lens is slightly loose or if there is any residual swelling or inflammation in the eye, it can lead to a sensation of shaking or instability when you try to focus. This is generally a temporary condition and should improve as the eye heals and the lens settles into its proper position. It’s important to give your eye some time to adjust after surgery, but if the shaking persists or worsens, you should definitely follow up with your ophthalmologist for further evaluation.

2. Intraocular Pressure (IOP) Concerns: The fluctuation in your eye pressure readings is concerning and warrants attention. After cataract surgery, it is not uncommon for patients to experience temporary increases in intraocular pressure. This can be due to several factors, including inflammation, the presence of residual viscoelastic material used during surgery, or even the body's response to the surgical trauma. An IOP of 14 mmHg is considered normal, but readings of 25 mmHg or higher indicate elevated pressure, which can be problematic. Elevated IOP can lead to complications, including glaucoma, especially if it remains uncontrolled. Since you do not have a history of diabetes or glaucoma, it is crucial to monitor this closely. Your ophthalmologist may prescribe medications to help lower your eye pressure, and they may want to perform additional tests to rule out any underlying issues.

In summary, while some symptoms you are experiencing can be part of the normal healing process after cataract surgery, the persistent shaking and elevated eye pressure should be addressed with your eye doctor. They may need to adjust your treatment plan or conduct further examinations to ensure your eyes are healing properly and to prevent any long-term complications.

If you have any further questions or if your symptoms worsen, please do not hesitate to reach out to your healthcare provider. It’s always better to err on the side of caution when it comes to your eye health. Wishing you a smooth recovery!
