Post-operative issues after circumcision?

Hello Doctor, I am 24 years old and underwent laser circumcision due to phimosis, which made it difficult for me to maintain hygiene.
I would like to ask:
1.
Is my extreme sensitivity of the glans due to being in a state of phimosis for a long time? How can I overcome this sensitivity?
2.
After the circumcision, I experienced stinging pain during urination and felt that my urine flow was slow on two occasions.
Could this indicate an inflammatory issue?
3.
What color should the glans be after circumcision to be considered normal, and what color would be considered abnormal? Thank you, Doctor.

Xiao Xiao, 20~29 year old female. Ask Date: 2020/06/21

Dr. Du Shixi reply Urology

Hello: 1) It will gradually improve in 1-2 weeks.
2) It is very common to experience pain in the glans during urination.
3) The glans is often darker in color, but it should improve in a few days.
Generally, doctors will schedule a follow-up appointment.
In the meantime, avoid erections.
Wishing you good health.

Post-circumcision concerns are quite common, especially regarding sensitivity, pain, and healing. It’s important to understand that circumcision, whether performed via laser or traditional methods, can lead to a variety of sensations and healing processes that may be uncomfortable or concerning. Let’s address your questions one by one.

1. Sensitivity of the Glans: Yes, it is quite common for the glans (the head of the penis) to become hypersensitive after circumcision, especially if you have been in a state of phimosis (the inability to retract the foreskin) for an extended period. When the glans is covered by the foreskin, it is protected from friction and exposure, which can lead to a lack of sensitivity. Once the foreskin is removed, the glans is exposed to air and friction, which can heighten sensitivity. To manage this, many men find that gradually exposing the glans to different sensations can help desensitize it over time. This can be done through gentle touch or by wearing loose-fitting underwear. However, if the sensitivity is causing significant discomfort, it may be worth discussing with your healthcare provider, as they can offer tailored advice or treatments.

2. Pain and Urination Issues: Experiencing pain during urination after circumcision is not uncommon, especially in the initial days following the procedure. This can be due to inflammation or irritation of the surgical site. If you feel a sharp pain or a burning sensation when urinating, it might indicate that the area is still healing or that there is some irritation. It’s essential to ensure that you are keeping the area clean and dry. If the pain persists or worsens, or if you notice any signs of infection (such as increased redness, swelling, or discharge), you should contact your healthcare provider for further evaluation. They may recommend pain relief options or assess for any potential complications.

3. Normal Healing Colors: After circumcision, the glans may appear red or pink, which is normal as it is healing. The color can vary depending on your skin tone and the extent of healing. Generally, a healthy healing site should not show signs of excessive redness, swelling, or discharge. If the color appears dark purple or black, or if there are signs of infection (such as pus or a foul odor), these could be indicators of complications that require medical attention. It’s also important to monitor for any changes in sensation or persistent pain, as these can also signal issues that need to be addressed.

In summary, while sensitivity and discomfort are common post-circumcision, it’s crucial to monitor your symptoms and maintain good hygiene. If you experience persistent pain, changes in color, or other concerning symptoms, don’t hesitate to reach out to your healthcare provider for guidance. They can provide reassurance, assess for any complications, and help you navigate the healing process effectively. Remember, healing takes time, and each individual’s experience can vary significantly.
