Postoperative Issues After Anal Fistula Surgery
Hello Dr.
Ke,
I recently had anal fistula surgery, and three days post-operation, I noticed some fecal matter appearing from the surgical opening while having a bowel movement.
Is this a normal situation or an abnormal one? Should I schedule a follow-up appointment, or can I just continue to observe the wound and keep it clean?

Dr. Ke Fangxu reply Surgery
The incision from the anal fistula surgery is left open and not sutured, so it is normal for a small amount of feces to be present in the wound.
This is why warm sitz baths are recommended after the surgery.
As long as there is no incontinence or significant bleeding, there is no cause for concern.
Just follow the scheduled follow-up appointments with your physician.

More Info
After undergoing anal fistula surgery, it is not uncommon for patients to have concerns about their recovery, especially regarding bowel movements and the surgical site. In your case, noticing stool coming from the surgical site three days post-operation can be concerning, but it is essential to understand the context of your situation.
Firstly, following anal fistula surgery, the surgical site may still be healing, and it is possible for some stool to pass through the area, especially if the fistula was not completely excised or if there is still some connection between the anal canal and the skin. This can happen if the surgical site is still inflamed or if there is a residual tract that has not yet healed. However, it is crucial to differentiate between normal healing and potential complications.
1. Normal Post-Operative Symptoms: After surgery, it is typical to experience some discharge from the surgical site, which may include mucus or even small amounts of stool. This is especially true in the early days following the procedure. The area may be sensitive, and the healing process can lead to some unusual sensations or appearances.
2. Signs of Complications: While some discharge can be normal, you should be alert for signs of complications. If you notice increased pain, swelling, redness, or a foul odor coming from the surgical site, these could indicate an infection or other issues that require medical attention. Additionally, if the amount of stool passing through the surgical site increases or if you experience any bleeding, you should contact your healthcare provider immediately.
3. Maintaining Hygiene: Keeping the surgical area clean is vital for preventing infections. After bowel movements, gently clean the area with warm water and mild soap, and avoid using harsh wipes or products that could irritate the skin. Pat the area dry with a clean towel or allow it to air dry to reduce moisture, which can promote bacterial growth.
4. Follow-Up Care: It is advisable to follow up with your surgeon or healthcare provider regarding your concerns. They can assess the surgical site and determine whether the discharge you are experiencing is within the normal range or if further intervention is needed. They may also provide specific instructions on how to care for the area during your recovery.
5. Observation: If the discharge is minimal and you are not experiencing any other concerning symptoms, you may choose to observe the situation for a few more days while maintaining proper hygiene. However, do not hesitate to reach out to your healthcare provider if you have any doubts or if your symptoms change.
In summary, while some discharge from the surgical site after anal fistula surgery can be normal, it is essential to monitor your symptoms closely. Maintain good hygiene, and do not hesitate to seek medical advice if you notice any signs of complications or if you have concerns about your recovery. Your healthcare provider is the best resource for personalized advice and guidance tailored to your specific situation.
