Chronic headache
Hello Doctor, I would like to ask the following question: For the past two to three months, I have been experiencing frequent headaches (about 1-2 times a month on average).
It usually starts with a feeling of pressure at the top of my head, which then extends to pain near my left temple.
The pain lasts for about three to four days and gradually alleviates.
During this time, I need to take painkillers, otherwise, I cannot function normally.
Additionally, I experience a sensation of swelling in my eyes, and occasionally I vomit.
This time, the headache was also accompanied by tinnitus, which I have already consulted an ENT specialist about.
The doctor prescribed medication, but I still have tinnitus.
I would like to know if I should see another specialist or if this is just due to fatigue.
Thank you for your response.

Dr. Jiang Junyi reply Neurology
Hello, based on your description, the symptoms resemble those of a migraine.
This type of headache lasts from 4 to 72 hours.
The characteristics of the headache include unilateral pain, which may feel throbbing or pulsating, often accompanied by nausea and vomiting, as well as sensitivity to light and sound.
Additionally, activities such as walking, climbing stairs, or head movements can exacerbate the pain.
Many patients require analgesics or preventive medications for relief.
In addition to pharmacological treatment, lifestyle adjustments are also crucial.
It is important to develop good habits, maintain a regular daily routine, ensure adequate but not excessive sleep, avoid overexertion, and practice relaxation techniques.
Some individuals may have specific triggers, such as certain foods, alcohol, drastic temperature changes, or moving in and out of air-conditioned spaces during hot weather.
Avoiding these situations can help prevent migraines.
I recommend consulting a neurologist to discuss evaluation and treatment options.
Wishing you good health and peace of mind.

More Info
Chronic headaches can significantly impact your quality of life, and understanding their symptoms, causes, and treatment options is crucial for effective management. Based on your description, it seems you are experiencing recurrent headaches characterized by a sensation of pressure at the top of your head, which then radiates to the left temple area. This pain lasts for several days and is accompanied by symptoms such as eye swelling, occasional nausea, and tinnitus (ear ringing).
Symptoms of Chronic Headaches
Chronic headaches can manifest in various forms, including tension-type headaches, migraines, and cluster headaches. The symptoms you describe—pressure-like sensations, temple pain, and associated nausea—are often indicative of a migraine. Migraines can also be accompanied by aura symptoms, which may include visual disturbances or sensory changes. The fact that you experience nausea and vomiting during these episodes further supports the possibility of migraines.
Possible Causes
1. Tension-Type Headaches: These are the most common type of headaches and are often triggered by stress, poor posture, or prolonged computer use. They typically present as a dull, aching sensation and can last from 30 minutes to several days.
2. Migraines: Migraines are characterized by intense, throbbing pain, often on one side of the head. Triggers can include hormonal changes, certain foods, stress, and environmental factors. The presence of nausea and vomiting, as well as the aura symptoms, aligns with migraine characteristics.
3. Cervicogenic Headaches: These headaches originate from issues in the cervical spine (neck) and can cause referred pain to the head. Poor posture, especially from prolonged computer use, can contribute to this type of headache.
4. Medication Overuse Headaches: Frequent use of pain relief medications can lead to rebound headaches, where the headache returns as the medication wears off.
5. Other Medical Conditions: Conditions such as sinusitis, hypertension, or even more serious issues like intracranial pressure changes could also be contributing factors.
Treatment Options
1. Lifestyle Modifications: Since you mentioned prolonged computer use, consider taking regular breaks to reduce eye strain and neck tension. Ergonomic adjustments to your workspace can also help.
2. Stress Management: Techniques such as mindfulness, yoga, and regular physical activity can reduce stress levels, which may help alleviate headache frequency and intensity.
3. Medication: Over-the-counter pain relievers like ibuprofen or acetaminophen can be effective for managing acute headache pain. However, if you find yourself relying on these medications frequently, it may be worth discussing preventive medications with your doctor.
4. Consultation with Specialists: Given the complexity of your symptoms, especially with the presence of tinnitus and the need for medication to manage headaches, it may be beneficial to consult a neurologist. They can provide a comprehensive evaluation and may recommend imaging studies (like an MRI) to rule out any underlying conditions.
5. Physical Therapy: If your headaches are related to neck tension or posture, physical therapy may help. A therapist can provide exercises to strengthen neck muscles and improve posture.
6. Alternative Therapies: Some individuals find relief through acupuncture, chiropractic care, or biofeedback therapy.
Conclusion
In summary, your symptoms warrant further evaluation, especially considering the recurrent nature of your headaches and the associated symptoms. While fatigue and stress can certainly contribute to headache frequency, it is essential to rule out other potential causes. A multidisciplinary approach, including lifestyle changes, medication management, and possibly consultations with specialists, will likely provide the best outcome for managing your chronic headaches. Always consult with a healthcare professional before making any changes to your treatment plan.
