Can they conceive?
Nine years ago, I underwent surgery for a pituitary gland enlargement.
I had not had my menstrual period for three years prior to the surgery, and I still have not had it since the operation.
Currently, my levels of estrogen, thyroid hormone, and other hormones are low, but I do not have any symptoms, so I am not taking any medication for supplementation.
Given this situation, do I have a chance of conceiving?

Dr. Lin Manying reply Obstetrics and Gynecology
The pituitary gland has many important areas that secrete hormones in the human body, among which LH, FSH, TSH, and Prolactin are related to ovarian function.
If both LH and FSH levels are low, ovulatory function may be affected.
Therefore, it is necessary to visit an outpatient clinic for a blood test to check the hormones related to ovarian function, which will determine whether medication can be used to stimulate ovulation.

After undergoing pituitary surgery for a pituitary adenoma, the potential for conception can be influenced by several factors, particularly hormonal balance. The pituitary gland plays a crucial role in regulating various hormones that are essential for reproductive health, including those that control the menstrual cycle and ovulation.
In your case, it appears that you have experienced amenorrhea (absence of menstruation) for several years, both before and after the surgery. This is often a sign of hormonal imbalance, particularly involving the hormones produced by the pituitary gland, such as luteinizing hormone (LH) and follicle-stimulating hormone (FSH), which are critical for ovarian function. Additionally, low levels of estrogen and thyroid hormones can further complicate the situation, as these hormones are vital for maintaining a healthy reproductive system.
The absence of menstruation and low hormone levels suggest that your ovaries may not be functioning properly, which can significantly reduce the chances of natural conception. However, it is important to note that every individual’s situation is unique. Some women with similar conditions have been able to conceive, especially if they receive appropriate hormonal treatment to restore balance.
Here are some considerations regarding your situation:
1. Hormonal Evaluation: It would be beneficial to undergo a comprehensive hormonal evaluation. This typically includes measuring levels of FSH, LH, estradiol, progesterone, and thyroid hormones. Understanding your hormonal profile can help determine the underlying causes of your amenorrhea and guide treatment options.
2. Hormonal Replacement Therapy: If your hormone levels are found to be significantly low, your healthcare provider may recommend hormone replacement therapy (HRT). This can help restore menstrual cycles and improve ovarian function, potentially increasing your chances of conception.
3. Fertility Treatments: If natural conception proves challenging, assisted reproductive technologies (ART) such as in vitro fertilization (IVF) may be options to consider. These methods can sometimes bypass hormonal imbalances by directly stimulating the ovaries and facilitating fertilization.
4. Consultation with Specialists: It is advisable to consult with both an endocrinologist and a reproductive endocrinologist. The endocrinologist can help manage your hormonal imbalances, while the reproductive specialist can provide insights into fertility treatments and options.
5. Lifestyle Factors: Maintaining a healthy lifestyle can also play a role in improving fertility. This includes a balanced diet, regular exercise, managing stress, and avoiding smoking and excessive alcohol consumption. These factors can positively influence hormonal health and overall well-being.
6. Monitoring and Follow-Up: Regular follow-up appointments are essential to monitor your hormonal levels and overall reproductive health. This will allow for timely adjustments to your treatment plan as needed.
In conclusion, while the absence of menstruation and hormonal imbalances can pose challenges to conception, there are potential pathways to explore. With appropriate medical evaluation and treatment, there may still be opportunities for you to conceive. It is crucial to work closely with your healthcare team to develop a tailored approach that addresses your specific needs and circumstances.
