Post-hemorrhoid surgery
Hello Dr.
Yu,
I would like to ask Dr.
Hsu about my condition.
I underwent minimally invasive surgery for hemorrhoids at a clinic in Kaohsiung in mid-May, and it has been two months since then.
I have been having regular bowel movements every day, but I still notice intermittent blood in my stool every two to three days.
The amount of blood is not significant, but there is a faint red color in my stool.
Is this considered normal? Whenever there is bleeding, I also experience a slight swelling sensation for a few hours after bowel movements.
My stools are not very thick; they are slightly thinner than a banana.
The consistency varies, sometimes resembling a ripe banana and other times an unripe banana.
Before the surgery, my stools were thicker than a banana, but now they are not and I still experience bleeding.
Could this be due to a narrow anal canal? During my follow-up appointment, the doctor mentioned that the skin around the wound is thinner, but it has not healed properly.
Should I go to the hospital for further examination?

Post-hemorrhoid surgery recovery can be a complex process, and it is not uncommon for patients to experience some bleeding during the healing phase. In your case, experiencing intermittent bleeding two months after a minimally invasive hemorrhoidectomy, along with a slight swelling sensation after bowel movements, raises several considerations.
Firstly, it is important to understand that after hemorrhoid surgery, the tissue around the anal area is still healing. The presence of blood in your stool, especially if it is bright red, typically indicates that the source of the bleeding is from the lower gastrointestinal tract, which includes the rectum and anus. This can be due to several factors:
1. Healing Process: The surgical site may still be healing, and any strain during bowel movements can cause minor bleeding. The tissue in the anal area is sensitive and can easily become irritated, especially if the stool is hard or if there is any straining involved.
2. Stool Consistency: You mentioned that your stool varies in consistency. If your stool is sometimes hard, it can lead to straining, which may exacerbate any existing irritation or injury to the surgical site. It is crucial to maintain a soft stool consistency to minimize trauma to the area. Incorporating a high-fiber diet, staying hydrated, and possibly using stool softeners can help achieve this.
3. Anal Fissures: If the bleeding persists, it could be a sign of an anal fissure, which is a small tear in the lining of the anus. This condition can occur post-surgery and can cause pain and bleeding during bowel movements.
4. Narrowing of the Anal Canal: You raised a concern about the possibility of a narrowed anal canal. This can occur due to scar tissue formation after surgery, leading to difficulty in passing stool and potential bleeding. If you are experiencing significant discomfort or difficulty with bowel movements, this is something to discuss with your healthcare provider.
5. Infection or Other Complications: Although less common, ongoing bleeding could indicate an infection or other complications related to the surgery. Signs of infection may include increased pain, swelling, redness, or discharge from the surgical site.
Given your symptoms, it is advisable to follow up with your healthcare provider. They may recommend a physical examination to assess the surgical site and determine if there are any complications such as fissures or strictures. Additionally, they may suggest imaging studies or a colonoscopy if they suspect any underlying issues that need to be addressed.
In summary, while some bleeding can be a normal part of the healing process after hemorrhoid surgery, persistent or recurrent bleeding warrants further evaluation. Maintaining a soft stool consistency, monitoring your symptoms, and consulting with your healthcare provider will be essential steps in your recovery. It is always better to err on the side of caution and seek medical advice if you have concerns about your healing process.
