Rubber band ligation for internal hemorrhoids?

Hello, the doctor found a small lump at the anal opening that could not be pushed back in.
After a visit to the hospital, a rubber band ligation was performed on July 3rd.
The day of the procedure, bowel movements were very uncomfortable, but afterwards there was not much sensation.
However, on July 11th, I noticed blood in my stool during one bowel movement.
Although there have been no further issues with blood in the stool and bowel movements have returned to normal, I still feel a foreign body sensation in the anus, similar to the feeling of a hemorrhoid protruding (a piece of tissue blocking the anal opening).
There is also a dull pain, akin to a soreness, and touching the area while showering causes pain.
Could this indicate that the hemorrhoids were not completely treated, or is it something else?

Mr. Deng, 20~29 year old female. Ask Date: 2020/07/19

Dr. Ke Fangxu reply Surgery

It is normal to experience a sensation of foreign body in the anus after rubber band ligation for hemorrhoids, as well as mild pain and bleeding.
Rubber band ligation is primarily a symptomatic treatment and not a complete cure for hemorrhoids.
If there are concerns about other issues, it is advisable to consult a physician for further evaluation.

More Info

After undergoing rubber band ligation for hemorrhoids, it is not uncommon for patients to experience a range of symptoms as part of the healing process. The symptoms you described, including the sensation of a lump at the anal area, discomfort during bowel movements, and occasional rectal bleeding, can be concerning but are often part of the normal recovery trajectory.
Rubber band ligation is a common and effective treatment for hemorrhoids, particularly for those that are prolapsed or causing significant discomfort. The procedure works by cutting off the blood supply to the hemorrhoid, causing it to shrink and eventually fall off. However, as with any medical procedure, there can be side effects and complications.

1. Post-Procedure Symptoms: After the ligation, it is typical to experience some discomfort, especially during bowel movements. This discomfort can be exacerbated by straining or passing hard stools. The sensation of a lump or "foreign body" feeling in the anal area can also occur as the tissue heals.
2. Bleeding: The presence of blood in the stool or on toilet paper can happen after the procedure. This is usually due to the healing process of the ligated hemorrhoid or irritation of the surrounding tissue. If the bleeding is minimal and resolves quickly, it is often not a cause for concern. However, if you experience significant bleeding or if it persists, it is essential to consult your healthcare provider.

3. Pain and Discomfort: The pain you describe, similar to a "scraped" feeling, could be due to irritation or inflammation of the anal tissue. This can happen as the area heals, especially if there is any residual hemorrhoidal tissue or if the ligation has caused some trauma to the surrounding area.

4. Incomplete Treatment: It is possible that the ligation did not fully address all hemorrhoidal tissue, especially if there were multiple hemorrhoids present. If you continue to feel a lump or experience pain, it may indicate that additional treatment is necessary.
5. Other Considerations: While your symptoms are consistent with post-ligation recovery, they could also indicate other issues, such as anal fissures, skin tags, or even a recurrence of hemorrhoids. It is crucial to differentiate between these possibilities, as they may require different management strategies.

Recommendations:
- Follow-Up Appointment: It is advisable to schedule a follow-up appointment with your healthcare provider or a gastroenterologist. They can perform a thorough examination to assess the healing process and determine if further treatment is needed.

- Symptom Management: In the meantime, consider using over-the-counter pain relief medications, warm sitz baths, and topical treatments to alleviate discomfort. Maintaining a high-fiber diet and staying hydrated can help prevent constipation and reduce straining during bowel movements.

- Monitoring Symptoms: Keep track of your symptoms, including any changes in bleeding, pain levels, or the sensation of the lump. This information will be helpful for your healthcare provider during your follow-up visit.

In conclusion, while your symptoms may be typical for someone recovering from rubber band ligation, it is essential to communicate any concerns with your healthcare provider to ensure proper healing and management of your condition.
