Post-operative itching and bleeding from the anus after hemorrhoid surgery?
Hello Doctor, I underwent rubber band ligation for internal hemorrhoids at the beginning of March.
Although I no longer feel a sensation of obstruction during bowel movements, I still notice significant bright red blood and experience a tearing sensation similar to that of an anal fissure.
Additionally, what troubles me the most is the occasional itching around the anal area.
Due to these ongoing issues, I have had two follow-up visits (the second with a different doctor).
The doctor used a scope to examine me and confirmed that there are no hemorrhoids left and nothing more to ligate.
They prescribed an anti-itch cream, but I am puzzled as to why I still experience these abnormal sensations.
Furthermore, could this be related to excessive cleaning? During both follow-up visits, the doctors mentioned that my anal area is too dry and advised me not to over-clean.
However, I only rinse with water while showering and do not engage in any additional cleaning methods.
I would appreciate your guidance on this matter.
Thank you.

After rubber band ligation for internal hemorrhoids, it typically does not cause symptoms such as itching or tearing sensations.
If itching persists, you can follow your doctor's instructions to use ointments, wear loose-fitting clothing, and keep the perineal area dry.
Avoid smoking, reduce the intake of dairy products and sugary beverages, and avoid foods that may cause allergies.
If there are any changes in the skin, you should consult a dermatologist or a colorectal specialist for further evaluation.

After undergoing a hemorrhoid banding procedure, it is not uncommon for patients to experience persistent symptoms such as itching, bleeding, and discomfort. These symptoms can be concerning, especially when they persist despite medical intervention. Let's break down the potential causes and management strategies for your situation.
Understanding Your Symptoms
1. Persistent Bleeding: The presence of bright red blood during bowel movements can be alarming. While some bleeding can be expected after hemorrhoid surgery, especially if there was significant tissue manipulation, ongoing bleeding may indicate other issues. Possible causes could include:
- Residual Anal Fissures: If you are experiencing a tearing sensation, it may suggest the presence of anal fissures, which can occur due to straining during bowel movements or passing hard stools.
- Inflammation: Post-surgical inflammation can lead to bleeding, especially if the area is irritated.
- Other Conditions: Conditions such as diverticulosis or colorectal polyps can also cause bleeding and should be ruled out if symptoms persist.
2. Itching: Anal itching can be caused by several factors:
- Skin Irritation: Over-cleaning or using harsh soaps can irritate the sensitive skin around the anus, leading to itching.
- Moisture: Excess moisture from sweat or inadequate drying after bathing can contribute to itching.
- Dietary Factors: Certain foods can exacerbate itching, such as spicy foods, caffeine, and alcohol.
Management Strategies
1. Dietary Adjustments: Ensure you are consuming a high-fiber diet to prevent constipation and reduce straining during bowel movements. This includes fruits, vegetables, whole grains, and plenty of fluids. Consider fiber supplements if necessary.
2. Gentle Hygiene Practices: While cleanliness is important, over-washing can lead to dryness and irritation. It is advisable to:
- Use mild, unscented soap and avoid scrubbing the area.
- Pat the area dry gently after bathing, rather than rubbing.
- Consider using a sitz bath to soothe the area without excessive cleaning.
3. Topical Treatments: If your doctor has prescribed an anti-itch cream, ensure you are using it as directed. Over-the-counter options such as hydrocortisone cream can also provide relief, but should be used sparingly and not for prolonged periods.
4. Follow-Up Care: Since you have already seen multiple doctors, it may be beneficial to seek a specialist's opinion, such as a colorectal surgeon or a gastroenterologist, particularly if symptoms do not improve. They may recommend further diagnostic procedures, such as a colonoscopy, to rule out other conditions.
5. Avoid Straining: If you experience pain during bowel movements, it is crucial to avoid straining, as this can exacerbate fissures and lead to further bleeding. If necessary, stool softeners can help ease bowel movements.
Conclusion
Persistent symptoms after hemorrhoid surgery can be frustrating and concerning. It is essential to communicate openly with your healthcare provider about your ongoing symptoms. They can help determine if further evaluation is needed and guide you on the best management strategies. Remember, while some discomfort can be expected post-surgery, significant bleeding or persistent pain should always be investigated to rule out other underlying conditions. Your health and comfort are paramount, so do not hesitate to seek further assistance if needed.
