Treatment duration for epididymitis?
I have been taking medication for epididymitis for six months.
I stopped the medication for a month in between, but it relapsed.
A few weeks ago, I stopped the antibiotics and have only been taking anti-inflammatory and gastric medications, and my condition has been good.
This week, the doctor said I could stop the remaining medications, but after not taking the medication for a day or two, I start to feel uncomfortable again.
I have a strange discomfort in the area from the groin to the testicle.
When I examine the epididymis, I can feel a small soft lump, and it is painful when I press on it.
Does this mean the inflammation hasn't fully resolved? The doctor mentioned that it might not be possible to eliminate it completely and that I may need to rely on medication to manage the pain.

Hello: In general, epididymitis does not require such a long treatment duration.
Please consult a urologist at a hospital for an examination.
Wishing you good health.

Epididymitis is an inflammation of the epididymis, a coiled tube located at the back of the testicle that stores and matures sperm. It can be caused by various factors, including bacterial infections, sexually transmitted infections, or non-infectious causes such as trauma or autoimmune conditions. Managing epididymitis typically involves a combination of antibiotics, anti-inflammatory medications, and supportive care.
In your case, it seems that you have been on antibiotics for six months, which is a considerable duration. The fact that you stopped the antibiotics for a month and then experienced a recurrence suggests that the underlying infection may not have been fully resolved, or there could be other contributing factors at play. It is not uncommon for symptoms to persist even after the initial treatment, especially if the infection was chronic or if there are residual effects from the inflammation.
When your doctor advised you to stop the remaining medications, it is likely that they assessed your condition and believed that the infection had sufficiently improved. However, the discomfort you are experiencing, particularly the "weird" sensations in the groin and testicular area, along with the tenderness upon palpation of the epididymis, indicates that inflammation may still be present. It is important to communicate these symptoms to your healthcare provider, as they may need to reassess your condition.
The presence of a soft, tender area on the epididymis could be a sign of ongoing inflammation or even a possible cyst or other benign condition. In some cases, the inflammation can lead to the formation of a spermatocele or an epididymal cyst, which can cause discomfort but may not require aggressive treatment unless they become painful or problematic.
Your concern about whether the inflammation can be completely resolved is valid. In some cases, especially with chronic epididymitis, complete resolution of symptoms may not be achievable. Instead, the goal of treatment often shifts to managing symptoms and preventing recurrence. This can include:
1. Continued Medication: Depending on your doctor's assessment, they may recommend a longer course of antibiotics or a different class of antibiotics if a specific pathogen is suspected. Anti-inflammatory medications can also help manage pain and swelling.
2. Supportive Care: This includes rest, applying ice packs to the affected area, and wearing supportive underwear to alleviate discomfort.
3. Follow-Up Care: Regular follow-ups with your healthcare provider are essential to monitor your condition. If symptoms persist or worsen, further diagnostic tests, such as an ultrasound, may be warranted to rule out other conditions.
4. Lifestyle Modifications: Avoiding activities that may exacerbate symptoms, such as heavy lifting or prolonged sitting, can be beneficial. Additionally, practicing safe sex and managing any underlying conditions (like urinary tract infections) can help prevent future episodes.
5. Alternative Therapies: Some patients find relief through physical therapy or pelvic floor exercises, which can help alleviate discomfort associated with chronic pelvic pain.
In conclusion, while it may be challenging to completely eliminate all symptoms of epididymitis, effective management strategies can significantly improve your quality of life. It is crucial to maintain open communication with your healthcare provider about your symptoms and treatment options. If you continue to experience discomfort, do not hesitate to seek further evaluation and support.
